Survey answers as Freshservice tickets
Raise a Freshservice ticket for each survey answer, or add it to a ticket as a note, through a Zap that runs the moment someone answers your survey.
From an answer in Usertick to Freshservice
Internal surveys and service surveys often surface a problem to fix. A Zap raises a ticket with the answer and the person's email, or adds the answer as a note on a ticket already open.
The Zap starts in Usertick. It can run on every new answer, once the AI has read one, when one turns urgent or when its status changes, and a workflow can hand over only the answers its conditions pick.
Pick a survey in the trigger, and each question arrives as its own field, so the score and the comment land in the Freshservice fields you choose. The answer's email, page, country, device and AI reading come with it.

What the Zap can do in Freshservice
Choose the action in Usertick, and Zapier opens with the trigger and the action already in place.
- 01Create a ticket
- 02Add a note to a ticket
Zapier's page for Freshservice listed 5 actions and searches on November 1, 2026. These fit survey answers best.

- Add Notes to a Ticket
- Create Requester
- Request Ticket Approval
Three Zaps to build with Freshservice
Each one starts on a Usertick trigger and uses an action from Zapier's page for Freshservice.
Set Freshservice urgency and impact on an IT complaint
Urgent Feedback raises a Freshservice ticket with Urgency and Impact set to High, the department that owns the service, AI theme as the subject and the Answers as the description.
Freshservice action: Create Ticket
Add a Freshservice requester from an onboarding survey
An employee onboarding survey starts New Response, and Create Requester adds the person with the Respondent email as primary email and the name question as first name, so a first ticket has a requester.
Freshservice action: Create Requester
Request Freshservice approval for a software request
An internal survey's software requests pass to a workflow's Workflow Step. After Create Ticket raises the ticket, Request Ticket Approval sends it to the approver you select, with the Answers already in the description.
Freshservice action: Request Ticket Approval
Zapier's free plan runs a Zap with one action. A Zap with a search step or a filter has more steps and takes a paid Zapier plan.
What Usertick sends to Zapier
Every trigger carries these fields, ready to map into Freshservice.
Attachments such as screenshots stay in Usertick, because a Zap can't open their links. The Response link opens the answer with its files in the inbox.
Connect Freshservice in three steps
Settings opens Zapier with the trigger and the action already in place.
- 1
Pick the action in Usertick
Open Settings, then Integrations, select Freshservice and choose what the Zap does there.
- 2
Sign in on Zapier
Sign in to Usertick from the Zap and choose the organization when you allow access.
- 3
Map the fields and turn it on
Pick the survey to watch, map the answer's fields to Freshservice and turn the Zap on.
To send only some answers, add a Zapier step for Freshservice to a workflow instead. The workflow's conditions decide which answers reach the Zap, and the workflow can be published once its Zap is on.

Create Ticket has a type setting. A report that something is broken fits the Incident type, and a request for new equipment or access fits Service Request, so two workflow steps can split the two.
Create Ticket requires a requester email, which Respondent email fills when Usertick has one. A survey sent by email, a signed-in page or a Contact details question gives Usertick that address.
Zapier comes with every plan in Usertick, and Free runs 1 Zap. On Zapier, a two-step Zap like this one runs on the free plan.
No. The Zap signs in with your Usertick account, and you choose the organization when you allow access. API keys are for Usertick's REST API.
Yes. Response Analyzed filters by type and urgency, Urgent Feedback runs once for each answer that turns urgent, and a workflow's Zapier step hands over only the answers its conditions pick.
Pick the survey in the trigger. Each question then arrives as its own field, such as Q1 with the score, ready to map into Freshservice.
A Zap that starts in Freshservice can use Usertick's actions. Update Response sets an answer's status, type or urgency, Add Comment adds a note your team sees in the inbox, and Find Response looks up an answer by email or ID first.
No. Attachments stay in Usertick, because a Zap can't open their links. The Response link field opens the answer, with its files, in the inbox.
