Enrollment survey template

Find the steps that slow enrollment, while they are still fresh

Four questions for new students, parents and guardians once enrollment is confirmed: how easy it was, which steps were unclear, how well they were kept informed and what would have made it easier. Use it as it is or change any question, and Usertick reads every written answer for you.

What this enrollment survey tells you

An enrollment experience survey asks new students, or the parents and guardians who enrolled them, how the path from application to a confirmed place went. This template takes about a minute: an effort score for applying and enrolling, the steps that were unclear, a rating for how well your team kept them informed, and what would have made it easier.

It is written for an email in the days after enrollment is confirmed, sent to a contact list of the people who enrolled that week. The same template runs as a link in the welcome email your admissions system sends, inside your own confirmation page when the Usertick script runs on that domain, or as a conversation that asks one question at a time.

Every written answer is read as it arrives and filed under a category from your education list, such as fees and payments or platform and access, with a type and an urgency. Results shows the average effort on the 1 to 7 scale and the share who found it easy, bars for each unclear step with the typed Other answers, and the communication rating, each against the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Customer effort

How easy was it to apply and enroll?

Why ask it
Enrollment is a task, so the right measure is effort. A scale from very difficult to very easy shows friction that a general satisfaction score would hide.
What you learn
The average on the 1 to 7 scale and the share who found it easy, with a hard-to-easy bar, compared with the period before.
Make it yours
Keep it first. In the email it can be answered inside the message, and the rest of the survey opens with that score kept.
Question 2 of 4 · Multiple choice · Optional

Which steps were unclear?

Why ask it
The steps that confuse people are the ones that cause calls, emails and missed deadlines. People pick every step that was unclear, or name another one under Other.
What you learn
Bars with the share and count for each step, with every typed Other answer listed beside the chart. Select a step to filter the whole page to the people who chose it.
Make it yours
Rename the steps to match your own process, such as Placement test or Transcript upload, and keep None of them as the last option.
Question 3 of 4 · Stars

How well did we keep you informed?

Why ask it
Between applying and enrolling, people wait on decisions, offers and deadlines. This rating shows whether your updates reached them in time.
What you learn
The average out of 5, a bar per rating and the share who rated it 4 or higher. Select the low ratings to see which steps those people found unclear.
Make it yours
When the rating drops, check the timing of your offer and deadline emails before you rewrite them.
Question 4 of 4 · Text · Optional

What would have made it easier?

Why ask it
One open question catches what the list of steps missed, in the words students and parents actually use.
What you learn
Usertick reads each answer, files it under a category from your list, marks it as an idea, a complaint or a question and flags the urgent ones, in any language.
Make it yours
Keep it optional, so someone in a hurry still sends the first three answers.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Send it in weekly batches

Export the week's confirmed enrollments as a CSV with an email and a name per line, import them to a contact list and send the survey to that list.

  2. 02

Ask the person who did the work

Send it to whoever filled in the forms: the student, or the parent or guardian when the student is under 16.

  3. 03

    Keep the skip on

Skip people who already received this survey is on by default, so a weekly send never asks the same family twice.

  4. 04

    Fix one step per intake

Pick the step with the largest share of unclear answers, rewrite its page or email, and check the share again after the next intake.
