Usertick
Packaging survey template

See your packaging through customers' eyes, the day the box is opened

Three questions for the day after delivery: the first impression on opening, a rating for protection, ease of opening, material and looks, and an optional photo of the packaging. Send it by email or share the link, and add an open question when you want written comments, which Usertick reads for you.

What this packaging feedback survey tells you

A packaging feedback survey asks customers about the box itself: whether it protected the item, how easy it was to open, how much material it used and how it looked. This template keeps it to three questions and under a minute: a face scale for the first impression, a grid that rates the packaging on four points and an optional photo.

It is written for an email the day after delivery, while the box is still in the recycling and the first impression is fresh. Usertick sends it to a contact list now or on a date up to 30 days ahead, and the face scale can be answered inside the email. The link also fits a QR code printed on an insert inside the box, which reaches customers at the moment they open it.

Results shows the faces from unhappy to happy with each share, a heat map for the four packaging points with row averages and a gallery of customer photos, each compared with the period before. Add an optional open question and Usertick reads every written answer as it arrives, filing it under a category such as shipping and delivery with a type and an urgency.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 ยท Emoji

What was your first impression when you opened the package?

Why ask it
The first look inside the box sets how the whole order feels, and a face scale captures it in one tap. It comes first so it can be answered inside the email.
What you learn
Results shows the faces from unhappy to happy, each with its share, against the period before, so a new box or insert shows its effect on the first impression.
Make it yours
Select the unhappy faces to see which of the four packaging points those customers rated low.
Question 2 of 3 ยท Matrix ยท Optional

How would you rate the packaging on the following?

Why ask it
Packaging is full of trade-offs: more padding protects better and uses more material, and a premium look can be harder to open. Rating four points on one grid shows which one to improve without hurting the others.
What you learn
A heat map shows how each row was rated, with row averages, so a low average for Using little material beside a high one for Protecting the item tells you where you could cut.
Make it yours
Add a row for anything you are testing, such as paper tape or a recyclable mailer, and drop rows you don't plan to change.
Question 3 of 3 ยท File upload ยท Optional

Share a photo of the packaging (optional)

Why ask it
A photo of a crushed corner or an overstuffed box shows your packing team more than a rating can, and it doubles as evidence for a carrier claim.
What you learn
Results shows a gallery of the uploaded photos, so you can scan how boxes arrive without asking customers to email you pictures.
Make it yours
Free allows 1 File upload question per survey, so this one runs on every plan. Keep it optional, or pair it with an open question about the packaging.

What the answers show

Results turns each question into numbers you can follow week by week.

How would you rate the packaging on the following?

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Ask the day after delivery

The first impression fades quickly, so send the survey the day after the order arrives. Usertick doesn't send on a delivery event, so schedule it from a list of delivered orders.

  2. 02

Print a code inside the box

A QR code on the packing insert reaches customers as they open the box. Its answers arrive as link responses with the rest.

  3. 03

Change one thing at a time

Swap one element, such as the filler or the tape, and compare the row averages with the period before. Two changes at once make it hard to tell which one helped.

  4. 04

    Add a question for comments

The template has no open question. Add an optional one after the grid so customers can explain a low rating, and Usertick reads each comment as it arrives.
