Pharmacy survey template

Hear how the counter went, from a QR code on the receipt

Four questions for after a pharmacy visit: overall satisfaction, how long customers waited, whether the instructions for their medicine were clear and what to improve. Print the QR code on the receipt or the bag, and Usertick reads every written answer for you.

What this pharmacy survey tells you

A pharmacy satisfaction survey asks customers about the visit around the prescription: the line, the counter and how well the pharmacist explained how to take the medicine. This template asks four questions in under a minute and never asks what the medicine was.

It is written for a QR code on the receipt, the bag or a sign by the counter, which opens the hosted survey page in the customer's phone browser. The same link works in your own pickup emails and on your website, and Usertick email can send it to a contact list.

Results shows the satisfaction average and the share who rated 4 or 5, the median wait with the middle half of answers, and the share who found the instructions clear. Every written answer is read as it arrives and filed under a category such as waiting times or staff and communication, with a type and an urgency.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

How satisfied were you with your visit today?

Why ask it
One overall rating opens the survey and gives each store a number to follow week by week. A tap on a 5-point scale is easy to do standing at the counter or on the way to the car.
What you learn
The average, a bar per rating and the share who rated 4 or 5, against the period before. Select the low ratings to read what those customers wrote in the last question.
Make it yours
Keep it first, since a single tap is the easiest way into a survey opened from a QR code.
Question 2 of 4 · Ruler · Optional

How long did you wait to be served?

Why ask it
A slow line can spoil a visit that went well at the counter. A ruler from no wait to 30 minutes or more lets customers drag to roughly how long it felt, without choosing from bands.
What you learn
The median wait and the middle half of answers drawn over the scale, compared with the period before, so a change in staffing shows up as a shorter or longer wait.
Make it yours
Keep the open top end at 30+ min, or raise it if your waits run longer at busy times of day.
Question 3 of 4 · Yes / No

Were the instructions for your medicine clear?

Why ask it
A clear explanation is part of the service at the counter, and a No is a conversation the pharmacist can have differently next time. The question asks about the explanation, never about the medicine itself.
What you learn
The share that said yes, against the period before. Select No to see whether those customers also waited longest.
Make it yours
Leave the name of the medicine out of the label, so customers have no reason to type it.
Question 4 of 4 · Text · Optional

Is there anything we could do better?

Why ask it
The open question catches what a rating can't: the line system, opening hours, an item that was out of stock or a pharmacist who took extra time.
What you learn
Each answer with its sentiment and theme, filterable by theme, with up to 8 themes and their trends. Usertick also marks each one as a complaint, an idea or praise and flags the urgent ones.
Make it yours
Write a placeholder that points to the wait, the counter and the store, and asks customers to leave out their name and their medicine.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Give each store its own survey

Results doesn't split answers by the tag on a link, so make one survey per store, or add a question that names the store, before you print the codes.

  2. 02

Put a code where people wait

A sign by the line reaches customers who throw the receipt away. Tag that link with ?utm_source=sign, so each answer keeps where it came from in workflows and Google Sheets.

  3. 03

Keep medicines out of the answers

Ask about the visit and the explanation, never which medicine or what it is for. Usertick's terms don't allow medical details in surveys, and Usertick doesn't sign business associate agreements.

  4. 04

    Hand complaints to the manager

Give each complaint an owner and a status in the inbox, and let a workflow post ratings of 1 or 2 to the store's Slack or Microsoft Teams channel.
