Grant report feedback survey template

Write the report funders want, by asking the people who read it

Four questions for funders and program officers that go out with your grant report: how clear it was, how convincing the evidence of impact was, whether the length was right and what they need next time. Use it as it is or change any question, and Usertick reads every written answer for you.

What this grant report feedback survey tells you

A grant report feedback survey asks funders what they made of your report while they still have it open. This template takes under a minute: stars for clarity, a 1 to 10 score for the evidence of impact, a check on length and one open question about the next report.

It is written to go out with the report, as a link in the email that delivers it, on the last page of the PDF or inside the report's page at its end when you publish it on your own website. Usertick can also send it by email to a list of your funder contacts a few days after the report goes out, with the clarity stars inside the message.

Every written answer is read as it arrives and filed under a category from your list, such as impact and updates or communication, with a type and a sentiment. Results shows the average clarity rating and the share who rated it 4 or higher, the average evidence score with a column per step and the share who found the length right, each compared with the report before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

How clear was the report?

Why ask it
A report that is hard to follow can hide strong results, and a clear one makes the program officer's own write-up for their committee easier.
What you learn
Results shows the average, a bar for each star and the share who rated 4 or higher, compared with the period before, so a new report format shows its effect at once.
Make it yours
Keep it first, so in a Usertick email the funder rates clarity inside the message and the rest of the survey opens with it kept.
Question 2 of 4 · Big scale

How convincing was the evidence of impact?

Why ask it
Clarity and evidence are different things: a funder can follow every page and still doubt the numbers. The 10-step scale gives room to show doubt without a flat no.
What you learn
Results shows the average and a column per step from 1 to 10, so a split between funders who are convinced and funders who are not is easy to see.
Make it yours
When scores sit in the middle, ask your program officer which measure their committee would trust, and add it to the next report.
Question 3 of 4 · Multiple choice

The length of the report was

Why ask it
Funders read many reports in a grant round, and the length question tells you whether to cut or to add before the next one.
What you learn
A donut shows the share and count of Too long, About right and Too short. Select Too long to see how those funders rated clarity and evidence.
Make it yours
When Too long leads, open the next report with a one-page summary and move the detail to an appendix.
Question 4 of 4 · Text · Optional

What do you need from our next report?

Why ask it
The open question gets the specific requests: a breakdown by site, a story from a participant, outcomes set against the plan you proposed.
What you learn
Usertick reads each answer as it arrives, files it under a category and a theme, marks it as an idea, a question or a complaint, and Results lists up to 8 themes with their trends.
Make it yours
Note each request in the grant's file, and answer it by name in the next report.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Put the link beside the report

Place the survey link right under the report link in your email, or on the last page of the PDF, so funders answer while the report is open.

  2. 02

    Ask everyone who reads it

Send it to each person at the funder who receives the report, since a program officer and a committee member often read for different things.

  3. 03

    Keep the survey the same

Use the same four questions with every report, so each card in Results compares this report with the one before.

  4. 04

    Show what changed

Open the next report with a short note on what you changed after the last round of feedback, such as a shorter summary or a new outcome measure.

Read the open answers from those funders, then pick one outcome measure to report more clearly next time. Ask the program officer which number their committee looks at first.
