Lab test survey template

Follow the lab visit through, from booking to the results

Four questions for once results are out: how easy the test was to book, how long patients waited at the lab, how the staff who took the sample did and how long results took. Use it as it is or change any question, and Usertick reads every response for urgency and customer risk.

What this lab patient survey tells you

A lab patient satisfaction survey follows a test from start to finish: booking it, waiting at the lab, the few minutes with the person who takes the sample, and the days until results arrive. This template asks four questions in about half a minute and never asks what the test was for or what it found.

It is written for the days after results are released, when patients can judge the whole wait. Send it by Usertick email to the patients whose results went out that week, or put the link in the message your own system sends when results are ready. The same survey also runs as a link on your website's lab page.

Results shows the booking ease score and the share who found it easy, the median wait at the lab with the middle half of answers, the staff rating with the share of 4s and 5s, and the share of each turnaround time. Each response gets an urgency and a customer risk reading, so the visits that need a call back are easy to find in the inbox.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Customer effort

How easy was it to book your test?

Why ask it
Booking is the first part of a lab visit that patients handle alone, usually with a referral and a phone number or a booking page. A 1 to 7 effort scale shows how much work it took to get a slot.
What you learn
The average on the 1 to 7 scale and the share who found it easy, with a hard-to-easy bar, against the period before.
Make it yours
Keep it first, so in the email patients tap a score inside the message and the rest of the survey opens with it kept.
Question 2 of 4 · Ruler · Optional

How long did you wait at the lab?

Why ask it
A booked slot doesn't always mean a short wait at the lab. A ruler from no wait to 60 minutes or more lets patients drag to roughly how long it was, without picking from bands.
What you learn
The median wait and the middle half of answers drawn over the scale, compared with the period before, so a change in staffing or opening hours shows up as a shorter or longer wait.
Make it yours
When walk-in and booked patients share one lab, add a question that asks which they were, then select each answer to compare their waits.
Question 3 of 4 · Stars

How would you rate the staff who took your sample?

Why ask it
The person who takes the sample is the patient's only face-to-face contact in a lab visit, and a nervous patient remembers how those few minutes went.
What you learn
The average, a bar per rating and the share who rated 4 or 5, against the period before.
Make it yours
With several lab sites, give each its own survey or add a question that names the site, since Results doesn't split answers by the tag on a link.
Question 4 of 4 · Multiple choice

How long did it take to get your results?

Why ask it
The wait for results can be the longest part of the experience, and it is what brings calls to the front desk. Bands from the same day to more than a week make the answer quick.
What you learn
A donut with the share and count of each band, and the stacked shares over time, so a slower turnaround shows up as a growing share of More than a week.
Make it yours
Set the bands around the turnaround you promise patients, so the share outside it is easy to read.

What the answers show

Results turns each question into numbers you can follow week by week.

How long did it take to get your results?

The share of each answer with its count. Select an answer to narrow the whole page to those people and read what else they said.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Send it after the results

Send the survey once results are out, so patients can rate the whole wait. Usertick doesn't know when your system releases results, so schedule the send from Usertick or put the link in your results message.

  2. 02

Ask about the visit, never the result

Leave test names and results out of every question. Usertick's terms don't allow health records or medical details in surveys, and Usertick doesn't sign business associate agreements.

  3. 03

Add an open question when needed

Add an optional text question at the end, such as What could have been easier?, and Usertick files each answer under a category such as waiting times or booking and appointments.

  4. 04

Check the wait after each change

When you change opening hours or add staff at the lab, compare the median wait with the period before in Results.
