Eye care questions that show how the exam and the new frames felt
Ask patients about booking the exam, how the test was explained, choosing frames and waiting for glasses. Each question below comes with when to ask it and what the answer tells you.
What eye care survey questions tell you
Eye care survey questions ask patients about the two halves of a visit to an eye care practice: the exam, from booking to how the test was explained, and the shop, from choosing frames to the fitting and the price. A patient can trust the exam and still walk out unhappy with the frames, and the second half decides whether they buy their next pair from you.
Ask about the exam a day after the visit, and about the glasses once they have arrived and been worn for a few days. Send by email from your appointment list, or put the survey's QR code on a card in the glasses case. Pick four to six questions; Usertick doesn't sign business associate agreements, so keep the questions on the service and leave prescriptions and eye conditions out of the answers.
Each written answer is read as it arrives and given a category, a theme, a sentiment and an urgency, so a pair that arrived late or a poor fitting reaches the store team before a request for more brands does.

The 14 eye care questions, with when to ask each
Grouped by what they tell you. Use them as they are or change the wording.
The eye exam
Start with booking and the explanation, then ask why patients came and what kept them away, so the exam side gets its own picture.
- 01Customer effort
How easy was it to book your eye exam?
A customer effort score for booking the exam, on a 7 point scale. Results shows the average and the share who found it easy; compare it before and after you add online booking.
- 03Dropdown
How long since your last eye exam?
Time since the last exam, from under a year to a first visit. Select a long gap in Results to filter the page and read what kept those patients away.
- 05Multiple choice
What did you come in for?
Why the patient came in, from a routine test to sunglasses. Results shows each share in a donut; consider removing A change in my vision, since it comes close to a health detail.
- 14Text
What stopped you coming in sooner?
The open question on what stopped the patient coming sooner. Keep the placeholder asking for no medical details, and read the themes for cost, time or booking.
- 02Stars
How well was the test explained as it went along?
A star rating for how the test was explained as it happened. Results shows the average and the share who rated 4 or higher, worth sharing with each person who runs exams.
- 04Yes / No
Did you leave knowing what your prescription means?
Asks whether the patient left knowing what the prescription means. A No points to the explanation at the end of the exam; ask about the explanation, never the prescription itself.
Choosing frames
These questions show how patients choose and how happy they are with the result, which shapes your range and your advice.
- 08Ranking
Rank what mattered when you picked your glasses
Patients put six factors in order, from price to how fast the glasses would be ready. Results shows the average place per item, which helps when you choose stock and lens options.
- 11Number
How many pairs did you try on?
How many pairs the patient tried on. Results shows the median and the middle half; a high median next to low frame scores suggests the range or the advice needs work.
- 06Big scale
How happy are you with the frames you chose?
Happiness with the chosen frames, from 1 to 10. Results shows the average and a column per step; ask it once the glasses have been worn for a few days.
- 07Text
What would improve how we help people choose frames?
The open question on helping patients choose. The placeholder asks for no medical details; filter by theme to see whether range, advice or time pressure comes up most.
Price, delivery and fitting
Ask about value, the wait for glasses and how often they are worn, the parts that decide where the next pair comes from.
- 10Multiple choice
How did the price feel for what you got?
How the price felt for what the patient got. Results shows each share in a donut; select Far too high to filter the page and read what those patients chose.
- 09Ruler
How many days did your glasses take to arrive?
Days until the glasses arrived, on a ruler up to 30. Results shows the median and the middle half, which you can compare with what your team promises at the counter.
- 12Matrix
Rate each part of your eye appointment
Rates booking, the test, choosing frames and the fitting in one grid. The heat map shows which part pulls the visit down, with an average for each row.
- 13Dropdown
How often do you wear them?
How often the patient wears the glasses. Select Hardly ever in Results to filter the page and read whether comfort, the frames or the price is behind it.
Tips for asking about eye care
Small choices that get honest, useful answers.
- 01
Ask after the exam and after the fitting
Ask about the exam the day after, and about the glasses a few days after they arrive, so each answer covers something the patient has lived through.
- 02
Keep prescriptions out
Usertick doesn't sign business associate agreements, so ask whether the prescription was explained and never what it says. Keep the placeholder that asks patients to leave out medical details.
- 03
Put the QR code in the case
A card with the survey's QR code in the glasses case reaches patients when they first wear the new pair.
- 04
Check delivery against promises
Compare the days the glasses took with the date given at the counter, and give every late pair an owner in the inbox.
