Vaccination questions that show how to make the next shot easier
Ask patients how the vaccination visit went, from booking and the wait to the staff and the record they took home. Each question below comes with when to ask it and what the answer tells you.
What vaccination clinic survey questions tell you
Vaccination clinic survey questions ask patients about a short visit with many small steps: booking the slot, finding the site, the wait, the person who gave the shot and the information they left with. A slow line or a missing record makes patients less likely to come back for the next dose or the next season.
Ask on the day of the visit or the day after. A QR code by the exit reaches walk-in patients at pop up sites and pharmacies, and an email or link reaches booked patients. Pick four or five questions and never ask which vaccine the patient had: Usertick doesn't sign business associate agreements, so keep the survey on the visit and keep the placeholder that asks for no medical details.
Each written answer is read as it arrives and given a category, a theme, a sentiment and an urgency. On every plan, a workflow can add each response to Google Sheets, which helps when you compare sites across a whole season.

The 14 vaccination visits questions, with when to ask each
Grouped by what they tell you. Use them as they are or change the wording.
Booking and arriving
Start with how easy the slot was to book and how long the visit took, the parts that decide whether patients come back.
- 01Customer effort
How easy was it to book your shot?
A customer effort score for booking the shot, on a 7 point scale. Results shows the average and the share who found it easy; compare it season by season as booking changes.
- 04Multiple choice
Where did you get it?
Where the patient got the shot, from a clinic to a travel clinic. Select a site in Results to filter the page and compare its wait and staff ratings with the others.
- 02Ruler
How many minutes were you in the clinic in total?
Total minutes in the clinic, on a ruler up to an hour. Results shows the median and the middle half, a fair measure of flow even when one session ran long.
- 10Multiple choice
How was the waiting area?
How the waiting area felt, from calm to uncomfortable. Results shows each share in a donut, which helps you set how many patients to book per slot.
The shot and the staff
These questions rate the people and the information, so each site knows what to keep and what to explain better.
- 05Stars
How gentle was the person who gave it?
A star rating for how gentle the person giving the shot was. Results shows the average and the share who rated 4 or higher, worth sharing with the staff who run the sessions.
- 13Matrix
Rate each part of the vaccination visit
Rates booking, arriving, the wait and the staff in one grid. The heat map shows which step pulls the visit down, with an average for each row.
- 09Big scale
How well informed did you feel before your turn?
How well informed the patient felt before the shot, from 1 to 10. Results shows the average and a column per step; low scores call for clearer information in the booking confirmation.
- 03Yes / No
Were you told what to expect afterward?
Asks whether the patient was told what to expect afterward. A No here tends to bring calls the next day, so add the missing information to the card patients take home.
- 08Yes / No
Did you get a record you can keep?
A yes or no on leaving with a record. Results shows the share that said yes, and a low share tells you to check how records are handed out or sent.
Coming back next time
Ask what brought patients in, what held them back and how they want to hear about the next one.
- 06Ranking
Rank what made you decide to come in
Patients put six reasons in order. Results shows the average place per item, which tells you whether reminders, a clinician's suggestion or travel plans bring people in.
- 14Text
What put you off coming in sooner, if anything?
An open question on what put the patient off coming sooner. Read the themes for cost, time or access, and keep the placeholder asking for no medical details.
- 07Text
What would make the visit smoother next time?
The open question for next time. The placeholder asks for no medical details; filter the answers by theme to see whether parking, the line or booking comes up most.
- 12Consent
May we remind you when your next one is due?
A checkbox the patient ticks to agree to a reminder, with words beside it that can link to your privacy policy. Put it before the reminder question and skip that question for patients who don't agree.
- 11Dropdown
How would you like your next reminder?
How the patient wants the next reminder. Results shows each share in a donut; Usertick doesn't send reminders, so use the answer in the system that does.
Tips for asking about vaccination visits
Small choices that get honest, useful answers.
- 01
Ask on the day
Put a QR code by the exit, so patients answer while the wait and the line are still fresh.
- 02
Never ask which vaccine
Usertick doesn't sign business associate agreements, so ask about the visit and leave the vaccine and any reaction out. Keep the placeholder that asks patients to leave out medical details.
- 03
Write for adults
Ask adult patients about their own visit, or parents and caregivers about the visit they came to, and never aim the survey at children.
- 04
Compare sites through a season
Give each site its own survey or QR code, so pop up sites and the main clinic each get their own wait time and ratings.
