Usertick
Employee benefits survey template

Spend your benefits budget where it counts, with answers from your own people

Seven questions on benefits: overall satisfaction, how well people understand them, a ranking of what matters, what goes unused, how easy enrolling is and how they compare with other employers. Use it as it is or change any question, and Usertick reads every written answer for you.

What this employee benefits survey tells you

An employee benefits survey asks people which benefits they value and which they ignore, so the budget follows what they use. This template asks seven questions in under two minutes: overall satisfaction, how well people understand their benefits, a ranking of six benefits, the ones never used, how easy enrolling is, how they compare with other employers, and one benefit to add.

It is written for the weeks before open enrollment or budget planning, as an email Usertick sends to every employee, with the satisfaction rating inside the email. You can also share the link in your team chat, or show the survey inside the benefits page of your intranet when the Usertick script runs there. For answers without names, turn on Anonymous responses and use the link or QR code, because a Usertick email ties each answer to the person's contact.

Every written answer is read as it arrives and filed under a category from your list, such as pay and benefits, with a type and an urgency. Results shows the average place of each benefit, the share who never used each one and the share who found enrolling easy, each compared with the period before.

The 7 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 7 · Stars

Overall, how satisfied are you with your benefits?

Why ask it
An overall rating gives you one number to compare before and after a change to the package.
What you learn
The average out of 5, a bar per rating and the share who rated 4 or higher, against the period before.
Make it yours
Keep it first, so the email opens on a question people can answer in one tap.
Question 2 of 7 · Multiple choice

How well do you understand the benefits available to you?

Why ask it
A benefit people don't understand can't be valued, so low satisfaction sometimes comes from poor explanation more than from the package.
What you learn
A donut with the share of each answer. Select Not well to see how those people rated and ranked their benefits.
Make it yours
If Not well and Not at all together take a large share, fix the benefits guide before you change the package.
Question 3 of 7 · Ranking · Optional

Rank these benefits by how much they matter to you

Why ask it
Ranking forces a choice, which tells you what to protect when the budget is tight.
What you learn
The average place of each benefit, so the order of what matters most is clear at a glance.
Make it yours
Use your own benefits in the list, with the names your employees know, and keep it to six or seven.
Question 4 of 7 · Multiple choice · Optional

Which benefits have you never used?

Why ask it
Unused benefits cost money and may need better explanation or replacing. People can pick every one they have never used.
What you learn
Bars with the share and count of each benefit never used. Compare it with the ranking: a benefit ranked high but rarely used may be hard to reach.
Make it yours
Use the same list as the ranking question, so the two answers line up benefit by benefit.
Question 5 of 7 · Customer effort

How easy is it to enroll in and use your benefits?

Why ask it
A benefit that is hard to enroll in or use loses its value, and the 1 to 7 effort scale measures that friction.
What you learn
The average on the 1 to 7 scale and the share who found it easy, with a hard-to-easy bar.
Make it yours
Select the hard end of the scale to read what those people wrote about the benefit they would add.
Question 6 of 7 · Multiple choice

How do our benefits compare with other employers' benefits?

Why ask it
How people see your package against other employers affects whether they stay and whether they recommend working with you.
What you learn
A donut with the share of each answer, from Much better to Much worse, plus Not sure.
Make it yours
Treat it as perception and pair it with your own market data before you change the package.
Question 7 of 7 · Text · Optional

What one benefit would you add?

Why ask it
One benefit to add, in people's own words, shows needs that your current list doesn't cover.
What you learn
Usertick reads each answer, files it under a category and a theme such as pay and benefits, marks requests as ideas, and Results lists the themes that come up most.
Make it yours
Count the requests by theme and put the top two in your next budget proposal.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Ask before the budget is set

Run it a month or two before open enrollment or budget planning, so the answers can still change the package.

  2. 02

    Use your own benefit names

Replace the six benefits with your own, using the same names in the ranking and the never used question.

  3. 03

    Keep health questions general

Ask about benefits, never about health conditions or treatment. Usertick's terms don't allow health records or medical details in a survey.

  4. 04

    Share what you changed

After open enrollment, tell people which answers led to a change, such as a new benefit or a clearer guide.
