Usertick
Manager feedback survey template

Give managers feedback they can use, from the people they lead

Three questions for team members about their manager: four statements on expectations, feedback, recognition and growth, how comfortable they are raising a problem, and one thing to do differently. Use it as it is or change any question, and Usertick reads every written answer for you.

What this manager feedback survey tells you

A manager feedback survey, often called upward feedback, asks team members to rate the person they report to. This template asks three questions in about a minute: four statements on what good managers do, how comfortable people are raising a problem, and one thing they would ask their manager to change.

It is written for a link shared twice a year, a few weeks before development conversations, so managers have the answers in time to talk about them. Turn on Anonymous responses and share it by link or QR code: Usertick then stores no user ID, traits or visitor ID with the answers, and keeps the country and a salted hash of the IP address. A survey sent through Usertick email ties each answer to the person's contact, so leave email out for this one.

Every written answer is read as it arrives and filed under a category from your list, such as managers and leadership, with a type and an urgency. Results shows a heat map of the four statements, the average comfort score and the written answers by theme, each compared with the round before.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Matrix · Optional

How much do you agree with these statements about your manager?

Why ask it
Four statements cover the parts of managing people that team members see every week: expectations, feedback, recognition and support for growth.
What you learn
A heat map per statement with its average, so a manager sees which of the four is strongest and which needs work.
Make it yours
Keep the same statements every round, so a manager can see whether a change since the last round showed up.
Question 2 of 3 · Big scale

How comfortable are you raising a problem with your manager?

Why ask it
Whether people raise problems early decides how much a manager hears at all, so it gets its own 1 to 10 scale.
What you learn
The average on the 1 to 10 scale and a column per step, so a cluster at the low end shows even when the average looks fine.
Make it yours
Select the low steps in Results to read what those people would ask their manager to change.
Question 3 of 3 · Text · Optional

What's one thing you'd ask your manager to do differently?

Why ask it
One concrete request is easier for a manager to act on than a general rating, and it gives the development conversation a starting point.
What you learn
Usertick reads each answer, files it under a category and a theme such as managers and leadership, marks it as an idea, a complaint or praise, and flags the urgent ones.
Make it yours
In a small team, share the themes with the manager and keep the individual answers with HR, since wording can point to the writer.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Duplicate it for each manager

Give each manager a copy of the survey, so each one reads Results for their own team. For a manager with very few reports, combine teams into one survey so answers are harder to trace.

  2. 02

    Tell people what is stored

Templates come with Anonymous responses off. Turn it on before you share the link or QR code, and say plainly what Usertick keeps: the country and a salted hash of the IP address.

  3. 03

    Close it before the talks

Set Closes at in the survey's availability a week or two before development conversations, so managers have time to read and plan.

  4. 04

Ask managers to name one change

A manager who tells the team what they will do differently after reading the answers shows that the survey matters.

Frequently asked questions

Twice a year, a few weeks before development conversations, gives managers time to act and lets you see change between rounds.

With Anonymous responses on and the survey shared by link or QR code, Usertick stores no user ID, traits or visitor ID with the answers, and keeps the country and a salted hash of the IP address. In a small team, the wording of an answer can still point to the writer, so decide with HR who reads the written answers.

Duplicate the survey for each manager, or add a first question that names the team and select its answers in Results. Selecting an answer filters the whole page to the people who gave it.

Read the written answers from the same round, then let the manager discuss one or two themes with the team. Offer coaching on the statement that scored lowest.

Usertick can send it to a contact list, but each answer then stays tied to the recipient's contact, even with Anonymous responses on. For upward feedback, paste the link into your own email or team chat.
