30-day new hire survey template

Hear how the first month went, while onboarding is still fresh

Four questions for new hires at day 30: how the month went, how ready they feel, what slowed them down and what the next new hire should know. Use it as it is or change any question, and Usertick reads every written answer for you.

What this 30-day new hire survey tells you

A 30-day new hire survey asks people what onboarding was like while they still remember the first week and already know the job. This template asks four questions in under a minute and a half: a rating for the month, how ready they feel on four basics, what slowed them down, and one piece of advice for the next person.

It is written for an email Usertick sends on day 30 to the new hire's work address. The rating sits inside the email, so a tap starts the survey, and the link opens on a short welcome screen titled One month in. The same template also runs as a link in your onboarding checklist or chat, or as a conversation, one question at a time.

Every written answer is read as it arrives and filed under a category from your list, such as tools and processes or growth and learning, with a type and an urgency. Results compares the month's rating with the period before, and selecting an answer such as Waiting for access shows what those new hires wrote.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

Overall, how satisfied are you with your first month?

Why ask it
One overall rating gives you a number to follow from one group of new hires to the next, and it can be answered inside the email in one tap.
What you learn
Results shows the average out of 5, a bar per rating and the share who rated 4 or higher, against the period before.
Make it yours
Keep it first and required, so the email opens on a question new hires can answer in a second.
Question 2 of 4 · Matrix · Optional

How ready do you feel on each of these?

Why ask it
Four basics decide whether a new hire can work on their own after a month: equipment, someone to ask, a clear role and knowing the team. Not yet, Partly and Fully is quicker to answer than a long scale.
What you learn
A heat map per item with its average, so a row such as Having a buddy or mentor that sits on Not yet stands out against the rest.
Make it yours
Replace a row with what your own onboarding promises, such as a first project or a meeting with leadership.
Question 3 of 4 · Multiple choice · Optional

What slowed you down the most?

Why ask it
Delays in the first month usually come from the setup around the new hire, and the people who just went through it can name them. They can pick more than one, or type their own.
What you learn
Bars with the share and count of each reason, with typed Other answers listed beside them. Select Waiting for access to read what those new hires said about the rest of the month.
Make it yours
Add the delays your managers mention most, such as a security training or a missing budget code, and keep Nothing, it went well as an option.
Question 4 of 4 · Text · Optional

What should the next new hire know on day one?

Why ask it
Advice for the next person is easy to write after a month and turns straight into material for your onboarding guide.
What you learn
Usertick reads each answer, files it under a category and a theme, marks it as an idea, a question or a complaint, and flags the urgent ones. Results lists the answers with their sentiment and the themes that come up most.
Make it yours
Copy the most useful answers into your welcome document and tell new hires that the advice came from people who joined before them.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Schedule the send on day one

Usertick doesn't send a survey on its own when someone reaches a date. When a new hire starts, create a send to their address and schedule it for day 30, since a send can be scheduled up to 30 days ahead.

  2. 02

Pair it with the first week check

Ask about equipment and the welcome on the Friday of week one, and about readiness at day 30, as two short surveys, so each one stays quick.

  3. 03

Tell new hires who reads it

Answers sent through Usertick email stay tied to the person's contact. Say in the email that the people team reads the answers and follows up, so new hires know what to expect.

  4. 04

Fix the top delay before the next start

Look at the most chosen reason in the delays question each month, fix it before the next group starts, and watch whether its share drops.
