Training feedback survey template

Learn what people take from a training, and what to teach next

Four questions for the end of a session: how useful it was, whether people can apply it this week, the format they prefer and the next topic. Use it as it is or change any question, and Usertick reads every written answer for you.

What this training feedback survey tells you

A training feedback survey asks employees right after a session whether it helped them do their work. This template asks four questions in about 75 seconds: a usefulness rating, four statements on relevance, use, the trainer and the length, two format choices for next time, and the topic to train on next.

It is written for the end of the session or the next morning, as an email Usertick sends to the people who attended, with the usefulness rating inside the email. In a room, show the QR code on the last slide so people answer before they leave; for an online session, post the link in the call chat. It can also open as a popup on your internal learning site when the Usertick script runs there.

Every written answer is read as it arrives and filed under a category from your list, such as growth and learning, with a type and an urgency. Results shows the average rating, the share who rated 4 or higher and a split bar for each format pair, compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

How useful was this training?

Why ask it
A single usefulness rating is the number you compare across sessions, trainers and topics. It asks about the employee's own work, which says more about a training than whether the session was fun.
What you learn
The average out of 5, a bar per rating and the share who rated 4 or higher, compared with the period before.
Make it yours
Duplicate the survey for each course, or add a first question that names the session, so Results can show one training at a time.
Question 2 of 4 · Matrix · Optional

How much do you agree with these statements?

Why ask it
Four statements separate the reasons a training lands or misses: the topic, the chance to use it, the trainer and the length.
What you learn
A heat map per statement with its average, so a session with a strong trainer and a weak I can apply it this week row shows the gap between knowing and doing.
Make it yours
Keep the same rows for every session, so you can compare trainings with each other.
Question 3 of 4 · This or that

For the next training, which format would you prefer?

Why ask it
Two quick either-or picks tell you how to build the next session: live or self-paced, more practice or more theory.
What you learn
A split bar for each pair, Live session against Self-paced and More practice against More theory, with the share on each side.
Make it yours
Replace a pair with a choice you are really weighing, such as morning or afternoon, or in person or remote.
Question 4 of 4 · Text · Optional

What topic should we train on next?

Why ask it
People who just finished a session know what they still miss, so the next topic comes from them.
What you learn
Usertick reads each answer, files it under a category and a theme such as growth and learning or tools and processes, and marks requests as ideas. Results groups the answers into up to 8 themes with their trends.
Make it yours
Read the themes before you plan the next quarter's training calendar, and say which topics came from the survey.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Ask before people leave the room

Show the QR code on the last slide and leave two minutes for it, while the session is fresh in everyone's mind.

  2. 02

Name the session in every round

Results and the inbox don't split answers by link tag, so use one survey per course, or a first question that names the session, to compare them.

  3. 03

Check use a few weeks later

Send a second short survey after a month asking whether people used what they learned and what got in the way.

  4. 04

Tell the next group what changed

Open the next session with one change that came from earlier answers, such as a shorter format or more exercises.

Frequently asked questions

At the end of the session or the next morning. Ask whether people used it in a second short survey a few weeks later.

Yes. Show or print the survey's QR code, and people answer on their phones on the hosted survey page.

Duplicate the survey per course, or add a first question that names the course or the trainer, and select an answer in Results to filter the whole page to that session.

Select the low ratings in Results and read what those people wrote about the next topic. A weak relevance row points to the wrong audience, and a weak length row points to the format.
