Offsite feedback survey template

Plan a better next offsite, with answers from the last one

Four questions for the days after a team offsite: how it went overall, the balance of work and social time, travel, venue, food and schedule, and what the next one should include. Use it as it is or change any question, and Usertick reads every written answer for you.

What this offsite feedback survey tells you

An offsite feedback survey asks the team how the days away went while the trip is still fresh. This template asks four questions in about 75 seconds: an overall rating, the balance between work sessions and social time, ratings for travel, venue, food and schedule, and what to include next time.

It is written for an email Usertick sends within two days of getting back, with the rating inside the email so one tap starts the survey. You can also show the QR code at the closing session, post the link in the offsite chat, or let people answer as a conversation, one question at a time.

Every written answer is read as it arrives and filed under a category from your list, such as team and culture or workload and pace, with a type and an urgency. Results shows the average rating, the share for each balance answer and a heat map of the logistics, compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

Overall, how satisfied were you with the offsite?

Why ask it
One overall rating is the number to compare from one offsite to the next, and it can be answered inside the email.
What you learn
The average out of 5, a bar per rating and the share who rated 4 or higher.
Make it yours
Keep it first, so the email opens on a one-tap answer.
Question 2 of 4 · Multiple choice

The balance between work sessions and social time was

Why ask it
The split between work and social time is the planning call with no obvious answer, and three choices settle it quickly.
What you learn
A donut with the share of each answer. Select Too much work to see how those people rated the schedule and the offsite overall.
Make it yours
Keep About right in the middle, so the two ends are easy to compare. If the answers split evenly between both ends, the agenda may suit some teams and not others, so add a team question next time.
Question 3 of 4 · Matrix · Optional

How would you rate the following?

Why ask it
Travel, venue, food and schedule are the logistics that shape how the days felt, and each gets its own rating.
What you learn
A heat map per row with its average, so a long journey or a crowded schedule stands out from the rest.
Make it yours
Add a row for anything new you tried, such as the hotel or an activity, and drop rows you didn't organize.
Question 4 of 4 · Text · Optional

What should the next offsite include?

Why ask it
Ideas for the next offsite come straight from the people who will attend it.
What you learn
Usertick reads each answer, files it under a category and a theme such as team and culture or workload and pace, marks ideas and complaints, and flags the urgent ones.
Make it yours
Share the top ideas when you announce the next offsite, and say where they came from. Ideas that come up again and again, such as more time with other teams, belong on the agenda.
