Usertick
Product feedback survey template

Hear what users think of the product, sorted by the part they mean

Four questions behind a feedback button in your web app: which part of the product the feedback is about, how satisfied the user is with it, what happened and, when it helps, a screenshot. Use it as it is or change any question, and Usertick reads every written answer for you.

What this product feedback survey tells you

A product feedback survey gives users one place to say what works and what doesn't, at the moment they notice it. This template asks four questions in about a minute: the area the feedback is about, a satisfaction score for that area, what happened compared with what the user expected, and an optional screenshot of the screen in question.

It is written for a side button in your web app: a tab on the edge of the screen that stays on every page, so users open it when something gets in their way. The same script runs in single-page apps, and the survey can also open as a popup after a trigger or sit inside a help or settings page.

Every written answer is read as it arrives and filed under a category from your list, such as ease of use or features and gaps, with a type, a sentiment and an urgency. Results shows the share of each area, and selecting one narrows the satisfaction score, the written answers and the screenshots to that area.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Multiple choice

What is your feedback about?

Why ask it
Sorting starts with the user. One tap on the area tells you which team owns the feedback, and it frames the score that follows.
What you learn
A donut with the share and count of each area, with every typed Other answer listed, so you see whether bugs, speed or missing features take up most of the feedback this period.
Make it yours
Rename the options after your own product areas, such as billing, reports or the editor, and keep the list short enough to read at a glance.
Question 2 of 4 · Stars

How satisfied are you with this part of the product?

Why ask it
A score tied to the area gives each part of the product its own number. A user reporting a bug can still be happy with the rest, and the score shows how much the problem weighs.
What you learn
The average on the 1 to 5 scale, a bar per rating and the share who gave 4 or 5, compared with the period before. Select an area in the first question to see the score for that area alone.
Make it yours
Keep the words this part of the product in the label, so the score follows the area the user picked instead of the whole product.
Question 3 of 4 · Text · Optional

Tell us more. What happened, and what did you expect?

Why ask it
Asking what happened and what the user expected gets a bug report and a use case in the same answer, which is what an engineer or a designer needs to act on it.
What you learn
Usertick reads each answer as it arrives, gives it a category, a theme and a type such as bug, idea or complaint, and flags the urgent ones. Results lists the answers with their sentiment, filterable by theme, with up to 8 themes and their trends.
Make it yours
Call Usertick.identify() in your app with the user's ID, plan and email, so each answer is tied to an account your team can look up and write back to.
Question 4 of 4 · Screenshot · Optional

Add a screenshot if it helps (optional)

Why ask it
A screenshot shows the screen, the data and the error message that words leave out. The user captures the whole screen or picks one element, then marks it up with a pen, arrows, boxes and a tool that hides private areas.
What you learn
Results shows the screenshots as a gallery of files, and each one stays with its response in the inbox, beside the area, the score and the written answer.
Make it yours
Free allows 1 screenshot question per survey, so this template runs as it is on every plan; password and card fields are always blurred, and data-usertick-mask hides any other element you choose.

What the answers show

Results turns each question into numbers you can follow week by week.

How satisfied are you with this part of the product?

The average rating and the share who gave 4 or 5 stars, against the period before.

What is your feedback about?

The share of each answer with its count. Select an answer to narrow the whole page to those people and read what else they said.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Keep the button on every page

Set the show frequency to every visit, so the side button stays after a user's first answer. With the default, until answered, the tab disappears once a user has sent feedback.

  2. 02

Name the button for what it does

Label the side button with a short phrase such as Feedback or Report a problem, and pick the icon that matches, so users know what opens before they click.

  3. 03

Route each area to its owner

On every plan, a workflow posts bug reports to your engineering channel in Slack or Microsoft Teams, and from Starter it can also post pricing feedback to another channel and open a Jira issue with the screenshot attached, or a Linear issue.

  4. 04

Track every report to the end

Give each response an owner and a status in the inbox, and add a note with the fix, so anyone on your team can see what happened to a report.
