Usertick
Product feedback survey template

Get the full picture of your product, from the people who use it

Nine questions for active users once or twice a year: how long and how often they use it, satisfaction, ease and value, how they would feel without it, what to build next and whether they would recommend it. Usertick reads every written answer and files it under a theme for you.

What this product feedback survey tells you

A product feedback survey checks how customers use and rate your product as a whole. This template takes under two and a half minutes and covers tenure and frequency of use, satisfaction, four statements on needs, ease, reliability and value, life without the product, what to build next, NPS and role.

It is written for an email to active users once or twice a year. Import them as a contact list and send or schedule it from Usertick; the first question sits inside the email. For the second round, turn off Skip people who already received this survey. The link also works in your newsletter.

Results shows each answer against the period before: the satisfaction average with the share who rated 4 or higher, a heat map per statement, the share who would be very disappointed, and the NPS with its detractors, passives and promoters. Usertick reads both open answers and files each one under a theme with a type and a sentiment.

The 9 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 9 · Multiple choice

How long have you been using our product?

Why ask it
Tenure explains many of the other answers: newer users tend to rate ease, long-time users rate value and reliability.
What you learn
A donut with the share of each tenure band. Select More than a year to see how your longest customers score the product.
Make it yours
Keep the same bands from round to round, so the groups compare.
Question 2 of 9 · Multiple choice

How often do you use it?

Why ask it
Frequency separates daily users, whose ratings carry the most experience, from occasional ones who may be drifting away.
What you learn
A donut with the share of each frequency, over time as stacked shares. Select Rarely to see how those users would feel without the product.
Make it yours
Read it with your own usage data to see whether people describe their use the way your logs do.
Question 3 of 9 · Stars

Overall, how satisfied are you with our product?

Why ask it
Overall satisfaction is the single number to track between rounds, asked before the details so the details don't sway it.
What you learn
The average, a bar per rating and the share who rated it 4 or higher (CSAT), compared with the period before.
Make it yours
Keep the wording the same every round, so the CSAT compares cleanly.
Question 4 of 9 · Matrix · Optional

How much do you agree with these statements?

Why ask it
Four statements split satisfaction into needs, ease, reliability and value, so a drop in the overall score has a cause you can point to.
What you learn
A heat map with one row per statement and the row averages. The weakest row is where to look first.
Make it yours
Swap a row for one your team is debating, such as support or speed.
Question 5 of 9 · Multiple choice

How would you feel if you could no longer use our product?

Why ask it
How disappointed users would be without the product is a common product-market fit question. The very disappointed group marks the users the product truly serves.
What you learn
A donut with the share of each answer, over time as stacked shares. Select Very disappointed to see those users' roles and how often they use the product.
Make it yours
Read what the very disappointed group asks for in question 7 first, since they are the users to build for.
Question 6 of 9 · Text · Optional

What would you use instead if our product were no longer available?

Why ask it
The alternative users name is your real competitor, which can be another product, a spreadsheet or doing the work by hand.
What you learn
Each answer with its sentiment and theme, filterable by theme, with up to 8 themes and their trends.
Make it yours
Keep it optional, and count the products named in each round.
Question 7 of 9 · Text · Optional

What's the one feature or improvement you'd most like to see?

Why ask it
One improvement per user, asked plainly, gives a list of requests from the people who use the product today.
What you learn
Usertick reads each answer, gives it a category, a theme and a type such as an idea or a complaint, and marks the urgent ones.
Make it yours
On every plan, send the ideas to Jira or Linear with a workflow, one issue per theme.
Question 8 of 9 · NPS

How likely are you to recommend our product to a friend or colleague?

Why ask it
NPS after the details gives a recommendation score you can track from round to round.
What you learn
The score (promoters 9 to 10 minus detractors 0 to 6), a bar of detractors, passives and promoters, per device, and the score over time as a line.
Make it yours
Select the detractors and read their answers to question 7 to see what holds the score down.
Question 9 of 9 · Dropdown · Optional

Which best describes your role?

Why ask it
Role lets you read every answer by who gave it, since an engineer and a founder can rate the same product very differently.
What you learn
A donut with the share of each role, with typed Other answers listed. Select a role to filter every card to that group.
Make it yours
Keep it optional and last, and use the same role list as your welcome survey so the two compare.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Keep it for a regular check

A full product survey takes over two minutes, so send it once or twice a year and use short surveys in the product between rounds.

  2. 02

    Ask active users only

Import users who logged in during the last month. Former users belong in a churn survey.

  3. 03

Compare each round with the last

Results compares each card with the period before, so keep the questions the same between rounds and look at what moved.

  4. 04

Share a report with the team

From Pro, a scheduled PDF report sends the survey's results to your team, so everyone sees the same numbers after each round.
