Design feedback survey template

Make every design round land closer than the last

Three questions to send after each review meeting: how satisfied the client is with the design now, how the round went on creativity, brand fit, feedback and speed, and what the next round should focus on. Usertick reads every written answer and files it under quality of work or scope.

What this design feedback survey tells you

A design feedback survey turns the reactions in a review meeting into answers your designers can work from. This template asks three questions in about a minute: a rating for where the design is now, a grid for creativity, fit with the brand, how you handled feedback and the speed of the round, and one open question for the next round.

It is written for an email right after each review meeting, while the client still has the screens in mind. Usertick can send it to the client's reviewers with the first rating inside the email, or you can paste its link next to the files in your design tool, in the meeting notes or in the project channel. It also runs as a conversation, one question at a time.

Each answer to the last question is read as it arrives and filed under a category such as quality of work, scope and changes or communication, with a type and an urgency. Results compares the rating with the period before and draws the grid as a heat map, so a round where feedback handling slipped stands apart from one where the work itself missed.

Send it from Usertick to a contact list, with the first question inside the email. A tap on an answer opens the rest of the survey with that answer already kept. Usertick sends 50 emails a month on the free plan, or connect your own sender.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Stars

How satisfied are you with where the design is now?

Why ask it
It asks about the design as it stands today, so the rating tracks progress from round to round instead of giving a final verdict too early.
What you learn
The average on the 1 to 5 scale, a bar per rating and the share who rated 4 or higher, compared with the period before, so you can see whether ratings climb between the first round and the third.
Make it yours
Keep it first, since a star rating sits inside the email and one tap starts the survey with that answer kept.
Question 2 of 3 · Matrix · Optional

How did this round go?

Why ask it
A client unhappy with the design may really be unhappy with how the round ran. The grid splits the creative work from the process, so the fix goes to the right person on your team.
What you learn
A heat map with one row per item and the average for each row, so How we handled your feedback at Fair stands out next to Creativity at Very good.
Make it yours
Add a row for what your clients often raise, such as Accessibility or Use of photography, and keep the columns the same every round so the averages compare.
Question 3 of 3 · Text · Optional

What should the next round focus on?

Why ask it
The client names the priority for the next round in writing, which settles which comment from the meeting matters most when several people spoke.
What you learn
Each answer with its sentiment and theme, filterable by theme, with up to 8 themes and their trends, so repeated requests about one part of the design show across rounds.
Make it yours
Paste the answer into the brief for the next round, so the designer works from the client's own words.

What the answers show

Results turns each question into numbers you can follow week by week.

How satisfied are you with where the design is now?

The average rating and the share who gave 4 or 5 stars, against the period before.

How did this round go?

Each row's answers side by side, so the part that needs work stands out.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Send it the same day

Send it within hours of the review meeting, before the client's team talks it over and the reasons behind each reaction blur.

  2. 02

    Ask every reviewer

Send it to each person who gave feedback in the meeting. Each answer is tied to its contact, so you can see when the marketing lead and the founder want different things.

  3. 03

Use one survey for the project

Keep the same survey for every round, so the rating and the grid can be followed from round to round in one place.

  4. 04

    Tag each round

Add a tag to the link, such as ?utm_source=round-2, so workflows and Google Sheets keep the round with each answer.

Frequently asked questions

Right after each review meeting, the same day if you can. A wait of a week mixes this round's reactions with expectations for the next one.

Yes. Three questions take about a minute, and the grid and the open question are optional, so clients can answer each time without much effort.

Look at the grid first. A drop in Fit with your brand calls for another look at the brand inputs, while a drop in How we handled your feedback calls for a call about the process.
