Usertick
Workshop feedback survey template

Leave discovery with outcomes everyone agrees on

Four questions for the day after a discovery workshop: how it went, whether the right people were there, how clear the outcomes are and what you missed. Usertick reads every written answer and files it under a category from your list.

What this workshop feedback survey tells you

A workshop feedback survey checks whether a discovery session gave the client what they came for. This template asks four questions in under a minute: a rating for the workshop, whether the right people were in the room, how clear the outcomes are, and what you missed.

It is written for an email the day after the workshop, after attendees have slept on it and before the full findings are ready. Usertick can send it to every attendee with the first rating inside the email, and a text block above it can list the agreed outcomes. You can also paste its link into your workshop recap or onto the shared board you used in the session.

Usertick reads each answer to the last question as it arrives and files it under a category such as scope and changes or team and people, with a type and an urgency. Results shows each rating with the share who gave 4 or 5 and the share who said the right people were there, compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

Overall, how satisfied were you with the workshop?

Why ask it
An overall rating of the session tells you whether the format worked for this client, before you look at the details.
What you learn
The average on the 1 to 5 scale, a bar per rating and the share who rated 4 or higher, compared with the period before, so a new workshop format can be judged against the last one.
Make it yours
Keep it first, since a star rating sits inside the email and a tap opens the next questions.
Question 2 of 4 · Yes / No

Were the right people in the room?

Why ask it
Discovery fails quietly when a decision maker was missing. A plain yes or no catches it before the plan is built on half the picture.
What you learn
The share that said yes. Select No to read what those attendees think you missed, which often names the person or team.
Make it yours
Add skip logic so a No goes to a short question asking who was missing, since logic works on any answer on every plan.
Question 3 of 4 · Stars

How clear are the outcomes of the workshop?

Why ask it
A good workshop can still end with fuzzy outcomes. Rating clarity apart from satisfaction shows whether the findings need a clearer write-up.
What you learn
The average and the share who rated 4 or higher, with a bar per rating. Compare it with the first rating: a happy room with unclear outcomes means the summary has work to do.
Make it yours
List the agreed outcomes in the email above the question, so attendees rate the same list they will work from.
Question 4 of 4 · Text · Optional

What did we miss?

Why ask it
Attendees often remember a topic that never came up, a team that will be affected or a constraint nobody mentioned. One open question collects those while there is time to add them.
What you learn
Each answer with its sentiment and theme, filterable by theme, and Usertick marks each one as a question, an idea or a complaint and flags the urgent ones.
Make it yours
Add every gap to the findings document and credit the attendee who raised it, so the client sees the answer was used.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Ask the day after

A night's sleep gives attendees time to notice what was missing. Schedule the send for the next morning before the workshop starts.

  2. 02

    Send it to every attendee

Each person saw a different part of the business. Add everyone to the send, and the inbox shows which attendee gave each answer.

  3. 03

Hold the plan for the answers

Wait to write the project plan until you have read the answers to What did we miss?, and add a short call when the room was missing someone.

  4. 04

    Reuse it for every workshop

Keep one survey for all your discovery sessions, so a change to the format shows in the ratings over time.
