Campaign feedback survey template

Agree on what the results mean and on what comes next

Four questions for after the results meeting: how the client thinks the campaign did against its goal, how clear your report was, what surprised them and what to do next. Usertick reads every written answer and files it under a category from your list.

What this campaign feedback survey tells you

A campaign feedback survey records how the client reads the results, which can differ from what the dashboard says. This template asks four questions in about a minute: how the campaign did against its goal in the client's view, how clear the results report was, what surprised them, and what to do next.

It is written for an email after the results meeting, while the numbers are fresh and before the next brief. Usertick can send it to the client's team with the first question inside the email, or you can paste its link into your meeting recap or at the end of the report. It also runs as a conversation, one question at a time.

Each answer to the surprise question is read as it arrives and filed under a category such as quality of work or communication, with a type and an urgency, and each response gets a customer risk reading. Results shows the share of each verdict and each next step, and the report rating with the share who gave it 4 or 5, compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Multiple choice

How did the campaign perform against its goal?

Why ask it
Your numbers and the client's verdict don't always match: a campaign that met its click goal can still feel like a miss to a sales team. Asking for the verdict shows where the story needs work.
What you learn
A donut with the share and count for each verdict, with stacked shares over time across campaigns. Select Missed the goal to see the report rating and the next step those clients chose.
Make it yours
Keep it first, since a multiple choice question sits inside the email and one tap opens the rest of the survey.
Question 2 of 4 · Stars

How clear was the results report?

Why ask it
A clear report makes the results easier to defend inside the client's company. A low rating here can explain a gap between your numbers and their verdict.
What you learn
The average on the 1 to 5 scale, a bar per rating and the share who rated 4 or higher, compared with the period before, so a new report layout can be checked against the old one.
Make it yours
Change the label to name the report, such as the fall paid social report, when a client gets several.
Question 3 of 4 · Text · Optional

What surprised you in the results?

Why ask it
Surprises show where the client's expectations sat, and those expectations shape the goal for the next campaign.
What you learn
Each answer with its sentiment and theme, filterable by theme, and Usertick marks each one as praise, a question or a complaint and flags the urgent ones.
Make it yours
Bring the surprises to the planning meeting for the next campaign, and answer each question in writing first.
Question 4 of 4 · Multiple choice

What should we do next?

Why ask it
It ends with a decision, so the meeting turns into a next step the client has chosen in writing.
What you learn
A donut with the share and count for each next step. Select Pause for now to see what those clients said about the results and the report.
Make it yours
Treat Scale it up as the start of a new proposal, and call within a day when a client picks Pause for now.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Send it the day of the meeting

Schedule it for an hour after the meeting ends, so the client answers while the numbers are fresh.

  2. 02

    Ask the budget owner too

Send it to the person who owns the budget as well as your day-to-day contact. Their verdicts can differ, and the inbox shows each one.

  3. 03

Set their verdict beside your data

Put the client's verdict next to the campaign's actual numbers in your notes. Where they differ, the report or the goal needs another look.

  4. 04

    Tag each campaign's link

Add a tag such as ?utm_source=spring-launch to the link, so Google Sheets and webhooks keep the campaign name with each answer.

Frequently asked questions

Right after the results meeting, the same day. If there is no meeting, send it a day after the report goes out.

The client's verdict decides the next budget. Their view tells you whether the numbers landed and whether the goal itself was the right one.

Call them before the next brief, go through the report together and agree on the goal for the next campaign. In Results, select Missed the goal to read everything those clients wrote.

Results doesn't split answers by link tag. To compare campaigns side by side, use one survey per campaign, or add a first question that names the campaign and filter by its answer.
