Monthly questions that show how the account is really going
Ask clients each month how the work, the report and the partnership are going, and get sign off on a final delivery. Each question below comes with when to ask it and what the answer tells you.
What monthly client feedback questions tell you
Monthly client feedback survey questions ask clients how the month went: whether deadlines held, how useful the report was, what needs attention and how they feel about the partnership. A retainer client who is quietly unhappy tends to show it in small scores long before they bring it up on a call.
Send the survey a day or two after the monthly report goes out, by link in the report email or from Usertick to the client contacts on a list. Usertick has no recurring sends, so schedule each month's send, up to 30 days ahead. Pick four or five questions and keep them the same every month, so each card in Results compares with the month before.
Each written answer gets a category, a theme and a sentiment, and each response an urgency. On every plan, a workflow can post a low score to the account's Slack or Microsoft Teams channel, so the account manager calls before the next report.

The 13 delivery and reporting questions, with when to ask each
Grouped by what they tell you. Use them as they are or change the wording.
How the month went
Start with how the month felt, then the facts behind it, so a low mood score comes with a reason you can act on.
- 01Mood dial
How did this month feel?
A mood dial for the month as a whole. Results shows the share of each face, and the card compares with last month, so a slipping account shows up at a glance.
- 03Yes / No
Did we hit the deadlines we agreed?
A yes or no on the deadlines you agreed. Select No in Results to see how those clients rated communication and speed in the grid.
- 04Matrix
Rate us this month on each
Rates communication, quality, speed and proactivity in one grid. Track the row averages month by month to see which part of the service needs attention before the renewal talk.
- 05Multiple choice
What needs attention this month?
Asks the client to name one area for this month, with Nothing as an option. A rising share for Communication or Speed tells you where to focus next.
- 09Yes / No
Are the results moving in the right direction?
A yes or no on the overall direction of results. When a client rates the report highly and still answers No, the work needs to change before the report does.
The report itself
Ask whether the report explains the work, so good results don't get lost in a report the client only skims.
- 02Stars
How useful was the monthly report?
A star rating for the monthly report alone. It can be answered inside the email, so put it first when you send the survey by email.
- 07Big scale
How well do you understand what we did this month?
How well the client understands the work behind the report, on a scale of 1 to 10. A low average means the work is happening and the client can't see it.
- 10This or that
In the report you want more
Two pairs: numbers or narrative, and what happened or what is next. Use the split bars to reshape the report around what this client reads.
- 08Multiple choice
Which channels performed for you?
Lets the client tick each channel they think performed. Compare the bars with your own numbers; a gap shows where the report needs to explain results better.
How the partnership is going
These questions look past the month to the relationship, and the open question gives the client room to say what the scores can't.
- 12Emoji
How do you feel about the partnership right now?
A row of faces for the partnership as a whole. Ask it every month, so a client who moves from Good to Okay gets a call before the renewal talk.
- 11Customer effort
How easy is it to reach your account manager?
A customer effort score for reaching the account manager. Results shows the average and the share who found it easy; a falling share is worth fixing before renewal.
- 06Text
What should we do differently next month?
The open question for next month. Read the answers by theme, and turn each concrete request into a task for the account team.
Sign off on a delivery
Close a delivery with a signature in the same survey, so approval and feedback arrive together.
- 13Signature
Sign to approve the final delivery
Collects a signature to approve the final delivery, kept with the answer in the inbox. Add it only in the month a delivery closes. Signature is on every plan, 1 per survey on Free.
Tips for asking about delivery and reporting
Small choices that get honest, useful answers.
- 01
Keep the same questions every month
Each card in Results compares with the period before, which only works when the questions stay the same.
- 02
Schedule the next send ahead
Usertick has no recurring sends, so schedule next month's survey as soon as the date is within 30 days.
- 03
Ask every contact on the account
Send the survey to each person who reads the report. A head of marketing and the day to day contact can see the same month very differently.
- 04
Reply before the next report
Give every low score an owner in the inbox and answer it before the next report goes out, so the client sees the feedback changed something.
