Usertick
Client check-in survey template

Walk into every monthly call knowing what is on the client's mind

Three questions to send a few days before your monthly call: how the work feels this month, what the client wants more of and what they want to discuss. Usertick reads every written answer and flags the ones that need a reply before the call.

What this client check-in survey tells you

A client check-in survey takes the temperature of a retainer between the big reviews. This template asks three questions in under a minute: a dial from Poorly to Very well for the month, a list of things the client would like more of, and an open question that sets part of the agenda for your next call.

It is written for an email a few days before your monthly call, so the answers can shape the agenda. Usertick can send it to your client contacts on the date you pick, with a button that opens the survey, or you can paste its link into your own monthly update or your shared project channel. It also runs as a conversation, one question at a time.

Each answer to the last question is read as it arrives and filed under a category such as communication, deadlines and delivery or quality of work, with a type and an urgency, and each response gets a customer risk reading. Results shows the share of each face on the dial against the period before, so a client sliding from Well to Unevenly is visible before it turns into a renewal problem.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Mood dial

How is our work together going this month?

Why ask it
A five-step dial gives a quick, honest read on the month without asking the client to grade your agency. Its words, from Poorly to Very well, describe the working relationship the way clients talk about it.
What you learn
The faces from unhappy to happy, each with its share, compared with the period before. Select Poorly or Unevenly to read only what those clients want more of and want to discuss.
Make it yours
Keep the five stops so each month reads against the last, and change only the words if your clients speak differently.
Question 2 of 3 · Multiple choice · Optional

What would you like more of from us?

Why ask it
A list of concrete asks turns a vague feeling into something the account team can act on, and the client can pick several at once.
What you learn
Bars with the share and count for each option, with every typed Other answer listed and stacked shares over time, so a growing call for Data and reporting shows before it becomes a complaint.
Make it yours
Swap the options for services you could add to the retainer, such as Paid social or Conversion work, so each answer points to a next step.
Question 3 of 3 · Text · Optional

Is there anything you'd like to discuss on our next call?

Why ask it
The client sets part of the agenda, and you hear about a sensitive topic before the call instead of in the middle of it.
What you learn
Each answer with its sentiment and theme, filterable by theme. Usertick marks it as a question, a complaint or an idea and flags the urgent ones, so a request to review the contract reaches the account lead first.
Make it yours
Read the answers the day before the call and open the call with the client's topic.

What the answers show

Results turns each question into numbers you can follow week by week.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Send it on the same day each month

Usertick doesn't repeat sends on its own, so schedule each month's send up to 30 days ahead, or press Send again on last month's send, and turn off Skip people who already received this survey.

  2. 02

Ask three or four days ahead

Leave time to read the answers and change the agenda. A send on the morning of the call brings answers after it.

  3. 03

    Read the trend across months

One Unevenly month after a launch is normal. Two in a row from the same client is a reason for a call with someone senior at your agency.

  4. 04

    Give each answer an owner

Assign each response to the account lead in the inbox, with a status and notes, so nothing from the check-in waits until the next one.
