Employee satisfaction survey template

Find out how satisfied your people are, from pay to the company's direction

Nine questions on overall satisfaction, pay, benefits, workload, managers, tools, where people work and the company's direction, with an eNPS score. Use it as it is or change any question, and Usertick reads every written answer for you.

What this employee satisfaction survey tells you

An employee satisfaction survey asks employees how they feel about the concrete parts of their job, from pay and benefits to tools and workload. This template asks nine questions in about three and a half minutes: the team, an overall score, satisfaction with seven parts of the job, eight statements on pay, workload, fairness and direction, raising concerns, the benefits that matter most, an eNPS score and two open questions.

It is written for a company-wide link shared once a year, opening on a welcome screen. Turn on Anonymous responses in the survey's Settings tab and share it by link or QR code, so Usertick stores no user ID, traits or visitor ID with the answers; it keeps the country and a salted hash of the IP address. A survey sent through Usertick email ties each answer to the person's contact. The QR code reaches people who work away from a desk.

Every written answer is read as it arrives, in any language, and filed under a category from your list, such as pay and benefits or workload and pace, with a type and an urgency. Results shows the overall score, a heat map for each grid, the eNPS and the benefits people value most, and selecting a team filters the whole page to that team.

The 9 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 9 · Dropdown

Which team are you in?

Why ask it
The team lets you compare satisfaction across the company and give each manager the view that concerns their people.
What you learn
A donut with the share of each team, with typed Other answers listed. Select a team to filter the whole page to it.
Make it yours
Merge teams with only a few people into one option, since Usertick has no minimum group size for results.
Question 2 of 9 · Big scale

Overall, how satisfied are you working here?

Why ask it
One overall score is the number to compare from year to year, before the detail.
What you learn
The average on the 1 to 10 scale and a column per step, against the period before.
Make it yours
Select the low steps to see which parts of the job those people rated lowest in the next question.
Question 3 of 9 · Matrix · Optional

How satisfied are you with each of these?

Why ask it
Seven parts of the job, each rated on its own, show which one pulls overall satisfaction down.
What you learn
A heat map per row with its average, from Very dissatisfied to Very satisfied, so pay or tools stand out against the rest.
Make it yours
Rename rows to fit your company, such as a named office or a shift pattern, and keep seven or fewer.
Question 4 of 9 · Matrix · Optional

How much do you agree with these statements?

Why ask it
Eight statements go past satisfaction to the reasons: whether pay is fair and understood, whether workload fits normal hours, whether managers are consistent and whether leadership is clear.
What you learn
A heat map per statement with its average, compared with the period before, so a change in how well pay decisions are understood shows after you publish pay bands.
Make it yours
When a pay statement scores low, read it next to the pay row in the previous question to see whether the issue is the amount or the process.
Question 5 of 9 · Multiple choice

Do you feel comfortable raising concerns with your manager?

Why ask it
Being able to raise concerns with a manager decides whether problems reach anyone in time.
What you learn
A donut with the share of Yes, Somewhat and No. Select No to read those people's written answers.
Make it yours
If No is common in one team, talk with that team's manager and offer another route for raising concerns, such as HR directly.
Question 6 of 9 · Multiple choice · Optional

Which three benefits matter most to you?

Why ask it
Choosing the benefits that matter most tells you where to spend when the budget is limited.
What you learn
Bars with the share and count of each benefit, so the most valued ones lead.
Make it yours
Use your own benefits in the list, and run a benefits survey when you need detail on use and enrollment.
Question 7 of 9 · NPS

How likely are you to recommend working here to a friend?

Why ask it
eNPS gives a number to set next to your quarterly eNPS survey and earlier rounds of this one.
What you learn
The score, promoters 9 to 10 minus detractors 0 to 6, with a bar of detractors, passives and promoters and a score line over time.
Make it yours
Use the same wording as your eNPS survey, so the two numbers line up.
Question 8 of 9 · Text · Optional

What do you like most about working here?

Why ask it
What people like most tells you what to keep when you change other things.
What you learn
Usertick reads each answer, files it under a category and a theme such as team and culture, and marks it as praise, an idea or a complaint.
Make it yours
Share a few of these answers with leadership next to the scores, choosing ones that don't point to the writer.
Question 9 of 9 · Text · Optional

What one change would most improve your experience here?

Why ask it
One change per person gives you a list of what to work on, ranked by how often each theme appears.
What you learn
Usertick reads each answer, files it under a category and a theme such as pay and benefits or workload and pace, and flags the urgent ones. Results shows up to 8 themes with their trends.
Make it yours
Pick the top two or three themes and report on them before the next round.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Write your own welcome screen

Say what Usertick stores with Anonymous responses on and who reads the answers. Promise only what the settings do.

  2. 02

Share it by link or QR code

A survey sent through Usertick email ties each answer to the contact. Post the link company-wide and print the QR code for people who work away from a desk.

  3. 03

Set Opens at and Closes at in the survey's availability, so everyone gets the same window and late answers don't spill into the next period.

  4. 04

    Pair it with shorter surveys

Run this full survey once a year and short pulse or eNPS surveys in between, so you can see whether your changes moved the numbers.

Frequently asked questions

Once a year, with shorter pulse or eNPS surveys between rounds. Keep the questions the same, so Results compares each year with the one before.

With Anonymous responses on and the survey shared by link or QR code, Usertick stores no user ID, traits or visitor ID with the answers, and keeps the country and a salted hash of the IP address. Keep teams broad, since Usertick has no minimum group size for results.

About three and a half minutes. Only the team, the overall score, the concerns question and the eNPS are required, so people short on time can skip the grids.

Select a team in the first question and the whole page filters to it. Save a view for each team, and keep small teams merged into one option.

Share the overall picture within a few weeks, pick a few themes, give each an owner and report on progress before the next round.
