A strategy alignment survey asks employees whether the company's direction is clear to them and whether they see their part in it. This template asks three questions in about 75 seconds: six statements on the strategy, their contribution, values, how leadership explains decisions, being informed and working with other teams; how confident they are in the direction; and what would help them understand it.
It is written for a link shared after a strategy update or an all-hands meeting, or once a quarter. Show the QR code on the last slide of the all-hands so people answer before they leave, or post the link in your team chat the same day. For answers without names, turn on Anonymous responses; it holds for link or QR code surveys, while a survey sent through Usertick email ties each answer to the contact.
Every written answer is read as it arrives and filed under a category from your list, such as managers and leadership or team and culture, with a type and an urgency. Results shows a heat map of the six statements and the average confidence score, each compared with the period before, so you see whether an update helped.
