An employee recognition survey asks employees whether their work is noticed and fairly rewarded, and what kind of recognition means something to them. This template asks four questions in about 85 seconds: four statements on recognition, reward and speaking openly, how often people are recognized, how they like to be recognized, and what the company could do better.
It is written for a link shared once a year, or a month or two after a change to pay or bonuses, with Anonymous responses turned on. Share it by link or QR code, so Usertick stores no user ID, traits or visitor ID with the answers; it keeps the country and a salted hash of the IP address. A survey sent through Usertick email ties each answer to the person's contact. The chat layout asks the same questions one at a time.
Every written answer is read as it arrives and filed under a category from your list, such as pay and benefits or managers and leadership, with a type and an urgency. Results shows a heat map of the four statements, the share for each frequency and bars for each kind of recognition, compared with the period before.
