Usertick
Employee referral survey template

Get more referrals from your own team, by fixing what stops them

Four questions on your referral program: whether people know how it works, whether they referred anyone this year, which reward would motivate them and what stops them. Use it as it is or change any question, and Usertick reads every written answer for you.

What this employee referral survey tells you

An employee referral survey asks employees whether they know your referral program and why they do or don't use it. This template asks four questions in about 50 seconds: two yes or no questions on knowing the program and referring this year, the reward that would motivate them most, and what stops them from referring.

It is written for once a year, alongside your hiring plans, as an email Usertick sends to every employee with the first question inside the email, so one tap starts the survey. You can also share the link in your team chat, print the QR code for the office, or let people answer as a conversation, one question at a time.

Every written answer is read as it arrives and filed under a category from your list, such as tools and processes or pay and benefits, with a type and an urgency. Results shows the share who know the program, the share who referred someone and the share for each reward, each compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Yes / No

Do you know how our referral program works?

Why ask it
A program people don't understand gets few referrals, so the first answer separates an explanation problem from a motivation problem.
What you learn
The share that said yes, compared with the period before. Choose No to see what stops those people from referring.
Make it yours
If the share is low, explain the program at your next all-hands before changing the reward.
Question 2 of 4 · Yes / No

Have you referred anyone this year?

Why ask it
The share who referred someone this year is the baseline for any change you make to the program.
What you learn
The share that said yes. Select Yes to compare the rewards chosen by people who already refer with everyone else.
Make it yours
Run it at the same time each year, so the share is comparable from year to year.
Question 3 of 4 · Multiple choice

Which reward would motivate you most?

Why ask it
What motivates people differs from one company to the next, and a single choice shows what to offer in yours.
What you learn
A donut with the share and count of each reward, with typed Other answers listed.
Make it yours
List only rewards you could really offer, and keep the list to four or five.
Question 4 of 4 · Text · Optional

What stops you from referring people?

Why ask it
The open question finds the real barriers, such as unclear roles, a slow process or never hearing back about a referral.
What you learn
Usertick reads each answer, files it under a category and a theme such as tools and processes or pay and benefits, marks complaints and ideas, and flags the urgent ones.
Make it yours
Read the answers from people who know the program but haven't referred anyone, since their barriers are the ones a bigger reward won't fix.

What the answers show

Results turns each question into numbers you can follow week by week.

Which reward would motivate you most?

The share of each answer with its count. Select an answer to narrow the whole page to those people and read what else they said.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Send it with the open roles

Ask when you share the year's hiring plans, so people have real roles in mind as they answer.

  2. 02

    Close the loop on referrals

When answers mention never hearing back, reply to every referral within a week and tell the employee how it went.

  3. 03

    Explain before you pay more

If many people answer No to the first question, fix the explanation before you raise the reward.

  4. 04

    Ask again after a change

Run the survey again a few months after you change the reward or the process, and compare the shares with the period before.
