Employee well-being survey template

Check in on your team's energy, once a month, in 30 seconds

Four quick questions on energy this month, workload, how well people switch off after work and whether they know where to get support. Use it as it is or add an open question, and Usertick reads every written answer for you.

What this employee well-being survey tells you

An employee well-being survey is a short, regular check on how people are holding up, so strain shows in the numbers while you can still ease it. This template asks four questions in about 30 seconds: energy this month, whether the workload is too much, too little or about right, how well people switch off after work, and whether they know where to get support.

It is written for a link shared once a month in your team chat. Turn on Anonymous responses in the survey's Settings tab and share it by link or QR code, so Usertick stores no user ID, traits or visitor ID with the answers; it keeps the country and a salted hash of the IP address. A survey sent through Usertick email ties each answer to the person's contact. The chat layout asks the same questions one at a time, with a short reply after a score.

Results shows the energy faces with their shares, the workload split, the average for switching off and the share who know where to get support, each compared with the month before. The template has no open question; add one and Usertick reads each answer, files it under a category such as workload and pace, and flags the urgent ones.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Mood dial

How is your energy this month?

Why ask it
Energy is easier to talk about than stress or health, and it still shows when a month has been hard.
What you learn
Each face from Very low to Very good with its share, against the month before, so a month where Low grows stands out.
Make it yours
Keep it first and required, so every response carries a reading of the month.
Question 2 of 4 · Multiple choice

How is your workload?

Why ask it
Three answers tell you whether the load is the cause, in either direction, since too little meaningful work drains people too.
What you learn
A donut with the share of Too much, About right and Too little. Select Too much to see how those people rated their energy.
Make it yours
If Too much grows two months in a row, raise workload in team meetings before the next round.
Question 3 of 4 · Big scale

How well can you switch off after work?

Why ask it
Switching off after work is where a heavy workload shows up at home, and a 1 to 10 scale picks up small changes from month to month.
What you learn
The average on the 1 to 10 scale and a column per step, so a growing group at the low end is visible even when the average holds.
Make it yours
Read it with the workload answer: low scores next to About right point to habits such as late messages more than to the amount of work.
Question 4 of 4 · Yes / No

Do you know where to get support if you need it?

Why ask it
Knowing where to get support is the part HR can fix directly, by telling people where help is.
What you learn
The share that said yes, compared with the month before.
Make it yours
Set Redirect after submit to your support page, so everyone lands there 2.5 seconds after the thank-you screen.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Keep it about work

Ask about energy, workload and support, and leave out health conditions and treatment. Usertick's terms don't allow health records or medical details in a survey.

  2. 02

    Share the link every month

Usertick sends no recurring emails, so post the link in your team chat at the same time each month and keep the questions the same.

  3. 03

    Point people to support

Name your support options in the message that carries the link, and redirect to your support page after the survey, since the people who answer No need it most.

  4. 04

    Act on the trend

Look at three months together before you treat a change as real, and tell people what you did about it.
