Annual report feedback survey template

Find out what supporters took from your annual report

Four questions linked from your annual report: how much supporters read, what stuck with them, which formats they prefer and how much they trust the way you spend money. Use it as it is or change any question, and Usertick reads every written answer for you.

What this annual report feedback survey tells you

An annual report feedback survey asks supporters what they did with the report your team spent months on. This template takes about a minute: how much they read, what stuck with them, two quick choices between formats and a 1 to 10 score for trust in how you spend money.

It is written to be linked from the report itself, as a link or a QR code on the last page of the PDF, or inside the report's web page at its end. The same survey can open as a popup on your website for visitors who read the report page, or go out by Usertick email to your supporter list a few weeks after publication.

Every written answer is read as it arrives and filed under a category from your list, such as impact and updates or giving and donations, with a type and a sentiment. Results shows the share for each reading level, a split bar for each pair of formats and the average trust score with a column per step, against the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Multiple choice

How much of the annual report did you read?

Why ask it
How far supporters read changes how to read every other answer: trust from someone who read all of it means something different from trust from someone who skimmed.
What you learn
A donut shows the share and count of each answer. Select I skimmed it to see which formats those readers prefer and how much they trust your spending.
Make it yours
Keep I didn't open it for the times the survey goes out by email to supporters who may never have opened the report.
Question 2 of 4 · Text · Optional

What stuck with you?

Why ask it
What a supporter remembers is the part of the report that worked, whether it was a number, a photo or a single family's story.
What you learn
Usertick reads each answer as it arrives, files it under a category and a theme, and Results lists up to 8 themes with their trends, so the stories that stuck stand out from the rest.
Make it yours
Keep it optional, so readers who remember little still answer the format and trust questions.
Question 3 of 4 · This or that

Which do you prefer?

Why ask it
Two quick pairs settle next year's format debates with your supporters' own preference: a PDF or a web page, numbers or stories. Supporters who prefer stories and supporters who prefer numbers may need different versions of the same report.
What you learn
Results shows a split bar for each pair, so A PDF against A web page and Numbers against Stories are clear at a glance.
Make it yours
Add a third pair for a choice your team is weighing, such as Printed or Digital.
Question 4 of 4 · Big scale

How much do you trust how we spend our money?

Why ask it
Trust in how money is spent is what an annual report exists to earn, and a 1 to 10 scale lets supporters show how much of it the report built.
What you learn
Results shows the average and a column per step, against the period before. Choose the low steps to read what those supporters remembered from the report.
Make it yours
Compare trust between supporters who read all of it and those who skimmed, by selecting each answer to the first question in turn.

What the answers show

Results turns each question into numbers you can follow week by week.

How much of the annual report did you read?

The share of each answer with its count. Select an answer to narrow the whole page to those people and read what else they said.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Put the link where reading ends

Place the survey link or QR code on the last page of the PDF, and the survey inside the page at the end of the web version.

  2. 02

    Ask readers who finish

From Pro, the Finished reading trigger opens the survey once a visitor has read most of the report page, so the people asked are the ones who got to the end.

  3. 03

Tag the PDF and the web page

Add ?utm_source=pdf to the link in the PDF and ?utm_source=email to the one in your email, so each answer keeps where it came from for your sheet, webhook or the API.

  4. 04

    Change one thing next year

Follow the format most readers preferred, and say in next year's report that the change came from their answers.
