A website feedback form stays available on every page, so visitors can speak up the moment something breaks or an idea comes to them, without waiting to be asked. This template first asks what they want to share, from reporting a problem to leaving a compliment, then asks for the details, an optional screenshot and an email if they would like a reply.
It is written for a side button: a tab on the left or right edge of the page that opens the survey as a popup when a visitor selects it. Keep it on your whole site or in your web app. The tab is hidden on screens 480 px wide or narrower, so on phones it reaches tablet and desktop visitors only.
Results shows the share of each kind of message, every message with its sentiment and theme, the screenshots in a gallery and the emails in a table. Usertick reads each message as it arrives, marks it as a bug, an idea, a complaint, a question or praise, and flags the urgent ones, so a broken checkout reaches someone first.
