Consent and next steps survey questions

Closing questions that turn an answer into a next step

Ask for permission to follow up, quote or interview, and give every customer something useful to do when they finish. Each question below comes with when to ask it and what the answer tells you.

What consent and follow-up questions tell you

Consent and follow-up survey questions close a survey: they ask whether you may contact the customer, quote them, invite them to an interview or share their answers, and they offer a next step such as a guide or a review. Asking first keeps your follow-up welcome, and a clear record of who agreed saves guesswork later.

Put them at the end of any survey, after the questions it is for, and pick one or two consent questions and at most one call to action. Ask for a signature only when there are real terms to agree to, and show the terms right above it.

The consent answers stay with each response in the inbox, so whoever owns the response can see what the customer agreed to before reaching out. On every plan, workflows can send contacts to HubSpot, Klaviyo or Google Sheets.

The 16 consent and next steps questions, with when to ask each

Grouped by what they tell you. Use them as they are or change the wording.

Permission to follow up

Ask whether and how you may reach the customer, and collect contact details only from those who agree.

  2. 02Consent

Permission to follow up by email. Use it on website and link surveys where you don't have the customer's address yet, and pair it with the contact question.

  3. 03Consent

Would you join a short user interview?

Invites the customer to a short user interview. Add logic so a Yes leads to the contact question, and give each Yes an owner in the inbox.

  4. 06Consent

Would you like to hear about new features?

An opt-in for news about new features. Keep it apart from service messages, and add only the customers who agree to your product updates list.

  5. 15Contact

How can we reach you about your request?

A name and email for the request the customer just made. Results shows them in a table, so whoever owns the response can reply without searching.

Using what customers said

Ask before you quote, publish or share answers, and record a signature where terms apply.

  1. 04Consent

    Can we quote your feedback?

Permission to quote the customer's feedback. Ask it after an open question, and quote only the words of customers who agree.

  2. 14Consent

Can we use your answers in an anonymous case study?

Permission to use the customer's answers in a case study without their name. Ask it after the open questions, and state clearly what will be published.

  3. 05Consent

Can we share your answers with our research partner?

Permission to share answers with a research partner. Name the partner in the question text, and share only the answers of customers who agree.

  4. 13Signature

Sign to confirm you agree to the terms above

A signature to confirm agreement with the terms shown above it. Use it for studies, pilots or agreements where you need a signed record. Signature is on every plan, 1 per survey on Free.

A next step for every customer

End with one button that fits the survey, such as setup help after onboarding questions or a review request after a good score.

  1. 07Call to action

Want to see what is new?

A button to your latest changes or release notes. Use it at the end of a product survey, so a customer who just gave feedback sees what changed recently.

  2. 08Call to action

Need a hand getting set up?

Offers setup help. Show it after onboarding questions to users who found setup hard, and link it to your guide or a booking page.

  3. 09Call to action

Would you leave us a review?

Asks for a public review. Add logic so only customers with a high score see it, and link the button to the review page you want them on.

  4. 10Call to action

Want early access to new features?

Invites the customer to early access. Show it to customers who asked for new features in the survey, and link it to the sign up page for your early access program.

  5. 11Call to action

Curious what the paid plans include?

A button to your plans page. Show it at the end of a survey for customers on a free plan, using targeting by user traits from Starter.

  6. 12Call to action

Want to read the full guide?

Links to a full guide. Put it after questions about a topic the guide covers, so a customer who struggled gets the answer straight away.

  7. 16Call to action

Thanks. Want to see what we do with feedback like yours?

A thank you with a link to what you do with feedback, such as a changelog or a public roadmap. Put it last, so every customer ends on a next step.
