Accessibility survey template

Find where your site gets in the way, for people who browse with assistive tools

Three questions on the assistive tools people browse with, how well your site works with them and where it blocks them. Use it as it is or change any question, and Usertick reads every written answer for you.

What this accessibility survey tells you

An accessibility survey asks the people who use your site with assistive tools what they rely on and where the site fails them. This template takes about a minute: which tools they use, such as a screen reader, keyboard only or zoom, a score for how well your site works with those tools, and an open question about where it gets in their way.

It is written for a link from your accessibility statement, the page people open when they want to report a barrier. The same survey runs inside the page at the end of that statement, as a side button that stays on the edge of every page, or as a link in your support replies.

Usertick reads each written answer as it arrives, files it under a category from your list, such as ease of use or speed and reliability, and marks it as a bug, a complaint or an idea, with an urgency. Results shows the share of each tool and the average score, and selecting a tool narrows the page to the people who use it.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Multiple choice · Optional

Do you use any of these when you browse?

Why ask it
The tools people use decide which barriers they meet: a screen reader user hits unlabeled buttons, a keyboard user hits focus traps and a zoom user hits layouts that break when the text grows. The question is optional, so nobody has to say anything about themselves to answer the rest.
What you learn
Bars with the share and count of each tool, since people can pick several, and every typed Other answer listed below, such as a switch device or a reading extension. Choose a tool to see the score and the barriers from those users only.
Make it yours
Add the tools your support team hears about, and keep None of these, so people who browse without assistive tools can still give the score.
Question 2 of 3 · Stars

How well does our site work with the tools you use?

Why ask it
One score per person shows whether your site works with the tools they named, and it gives you a number to track after each fix.
What you learn
Results shows the average, a bar per rating and the share who rated 4 or higher, against the period before. Select a tool in question 1 to see its score on its own, so a site that works with zoom but fails with a screen reader shows it.
Make it yours
Read the score again after each accessibility release, and compare it with the period before to see whether the fix reached the people it was meant for.
Question 3 of 3 · Text · Optional

Where does our site get in your way?

Why ask it
Barriers are specific: a form field without a label, a menu that traps the keyboard, a video without captions. The open question gets the page and the element in the person's own words.
What you learn
Usertick reads each answer, gives it a theme and a type such as a bug or a complaint, and flags the urgent ones, such as a checkout that can't be finished with a keyboard. Filter the answers by theme to see which barrier comes up most.
Make it yours
Set the placeholder to Name the page and what happened, so your team can find the barrier without a reply.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Link it where barriers get reported

Put the link on your accessibility statement and on your contact page, so people find it at the moment something blocks them.

  2. 02

Send blocked tasks to a person

On every plan, a workflow posts urgent answers to Slack or Microsoft Teams, so a sign-in or a checkout that a keyboard can't finish reaches the team that owns it.

  3. 03

    Give every barrier an owner

Assign each response in the inbox and set its status as the fix moves along, so no reported barrier gets lost between teams.

  4. 04

Ask about tools and barriers only

Keep the questions on the tools people use and the places they get stuck. Leave out medical conditions: Usertick's terms don't allow collecting health details.

Yes. The survey uses dialog semantics, works with the keyboard alone on every question in this template, announces errors to screen readers and follows reduced motion and Windows high contrast settings. Try it with the tools your visitors use before you link it.
