Usertick
Donor feedback survey template

Learn what moved each gift, while the thank-you is still fresh

Three short questions for donors right after they give: how easy the donation was, what inspired it and how they want to hear what it made possible. Use it as it is or change any question, and Usertick reads every written answer for you.

What this donor feedback survey tells you

A donor feedback survey asks people who just gave how giving went and why they gave. This template keeps it to three questions and about 35 seconds: a score for the donation itself, what inspired the gift, and how the donor wants to hear what it made possible.

It is written for the days right after a donation, as a link in the thank-you email your donation platform sends or as an email Usertick sends to a list of recent donors, with the first question inside the message. On your website it can sit inside the thank-you page that follows the donation form, as long as that page runs on your own domain.

Every typed Other answer is read as it arrives and filed under a category from your list, such as giving and donations or impact and updates, with a type and a sentiment. Results compares the ease score with the period before and shows the share of each reason to give and each way donors want to hear back.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Customer effort

How easy was it to make your donation?

Why ask it
The donation form is the step every donor just finished, so the score is fresh. An effort scale from very difficult to very easy points to friction in the form itself, such as too many fields or a card that was declined once.
What you learn
Results shows the average on the 1 to 7 scale and the share of donors who found it easy, with a hard-to-easy bar, against the period before. Select the hard end to see what inspired those donors and how they want to hear back.
Make it yours
Keep it first, so in a Usertick email the donor taps a score inside the message and the rest of the survey opens with it kept.
Question 2 of 3 · Multiple choice · Optional

What inspired you to give today?

Why ask it
Knowing what moved a gift tells you which stories, events and asks to repeat. Donors can pick more than one reason, since a friend's ask and the cause itself often work together.
What you learn
Bars show the share and count of each reason, with every typed Other answer listed beside the chart. Over time, the stacked shares show when a campaign story or a matching gift starts to bring in more donors.
Make it yours
Name your own appeals and events in the options, such as the spring appeal or the annual gala, so each reason matches work your team actually did.
Question 3 of 3 · Multiple choice

How would you like to hear what your gift made possible?

Why ask it
Asking how a donor wants to hear back lets you send the impact update they will actually open, and I don't need updates tells you who would rather get less mail.
What you learn
A donut shows the share and count of each way to hear back. Select Letters by mail to see how many donors still want paper and what inspired them to give.
Make it yours
Drop any option you can't deliver, such as a short video, so every choice a donor makes is one your team can keep.

What the answers show

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Ask within a few days

Send the survey while the gift is still in mind. Schedule a Usertick send to your list of recent donors, up to 30 days ahead, or put the link in the thank-you email your donation platform already sends.

  2. 02

Keep a list of recent donors

Import each week's donors as a CSV into one list and send to that list. Skip people who already received this survey is on by default, so a donor who gives twice is asked once.

  3. 03

Carry the answer to your donor records

On every plan, a workflow adds each answer to a Google Sheet or runs a Zapier step that updates the donor in Salesforce, so the next impact update goes out the way they asked.

  4. 04

    Tag every link

Add ?utm_source=receipt or ?utm_source=newsletter to each link, so every answer keeps the place it came from for your sheet, webhook or the API.

Within a few days of the gift, while the donation form and the reason to give are still in mind. Send it to a list of that week's donors, or put the link in your thank-you email so each donor gets it with the receipt.

Usertick doesn't send a survey when a gift lands in another system. Put the survey link in the thank-you email your donation platform sends, or import recent donors as a list each week and schedule a send. A HubSpot workflow (on any plan, with HubSpot Professional or Enterprise) can also send the survey to each contact it enrolls.

Yes. In a Usertick email the ease scale sits inside the message, and a tap opens the rest of the survey with that score kept. The answer is stored when the donor submits.

Few enough to finish in under a minute on a phone. This template asks three and takes about 35 seconds, so a donor can answer straight from the thank-you email.

Open the Audience tab in Results, where each device shows its own ease score, so a donation form that fails on phones stands out. Then give through the form yourself on that device before the next appeal goes out.
