Nonprofit partnership survey template

Keep partnerships strong with a plain yearly check

Three questions for partner organizations once a year: how communication, shared goals, credit and reliability rate, how likely they are to recommend partnering with you and what to work on together next. Use it as it is or change any question, and Usertick reads every written answer for you.

What this nonprofit partnership survey tells you

A nonprofit partnership survey asks the organizations you work with how the partnership feels from their side. This template takes about a minute: a grid for communication, shared goals, sharing credit and reliability, a 0 to 10 recommendation score and one open question on what to do together next.

It is written for once a year, sent by Usertick to a list of your contacts at partner organizations, such as coalition members, referral partners and the agencies you deliver programs with. The same survey runs as a link in your own email or in a coalition newsletter. Send every partner organization the same survey, so their answers sit side by side on one Results page.

Every written answer is read as it arrives and filed under a category from your list, such as communication or programs and services, with a type, a sentiment and a customer risk reading. Results shows a heat map of the four parts of the partnership with row averages and the NPS with its promoters, passives and detractors, against the period before.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Matrix · Optional

How would you rate the partnership on the following?

Why ask it
Partnerships wear down on small things: late replies, goals that drift apart and credit left out of a press release. The grid asks about each one by name.
What you learn
A heat map shows each row from Poor to Excellent with its average, so Sharing credit fairly scoring below Reliability is visible before it turns into a dispute. Select Poor or Fair in a row to read what those partners wrote about working together next.
Make it yours
Rename Shared goals to the outcome you agreed on together, such as families housed this year.
Question 2 of 3 · NPS

How likely are you to recommend partnering with us to another organization?

Why ask it
Whether a partner would recommend working with you is the plainest test of the partnership's standing among organizations like theirs, and it gives you one number a year.
What you learn
Results shows the NPS (promoters 9 to 10 minus detractors 0 to 6), a bar of detractors, passives and promoters, and the score over time as a line. Select the detractors to see how they rated each row of the grid.
Make it yours
Move it first when you want partners to answer inside the Usertick email, since the grid shows a start button there.
Question 3 of 3 · Text · Optional

What should we work on together next?

Why ask it
The open question turns the survey into planning: partners name the project, the referral route or the joint funding bid they want next. A partner with nothing to suggest may also be telling you something about where the partnership is heading.
What you learn
Usertick reads each answer as it arrives, files it under a category and a theme from your list, marks it as an idea, a question or a complaint, and Results lists up to 8 themes with their trends.
Make it yours
Bring the ideas that come up more than once to your next partners' meeting.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Ask everyone who works with you

Add every contact at a partner, from the director to the caseworker who sends referrals, to one partners list, since each sees a different side of the work.

  2. 02

Send it at the same time each year

Pick a fixed month, such as before annual plans are set, so the NPS line gets one point a year and the answers can feed the plan.

  3. 03

    Call detractors within a week

On every plan, a workflow posts scores of 0 to 6 to your partnerships channel in Slack or Microsoft Teams, so someone calls that partner while the answer is fresh.

  4. 04

    Report back to partners

Share the main themes and what you will change with every partner who answered, before you send the next survey.

Frequently asked questions

Once a year, at the same time each year, and again after a large joint project ends. A survey after a joint project can ask about that project alone, with its own rows in the grid.

Everyone who works with your team: the director who signed the agreement and the staff who handle referrals or shared programs day to day. Send to a list of partner contacts, and each answer from a Usertick email stays tied to the person who gave it.

There is no fixed number to aim for. Compare the score with your last round and read what promoters and detractors wrote.

Ask that partner how they want to be named in reports, posts and funding bids, and agree on it in writing before the next joint announcement.

Yes, by link: turn on anonymous responses and share the link in your own email, since anonymous answers hold for link or QR code surveys. A Usertick email ties each answer to the partner's contact.
