Apprenticeship questions that show who is on track to finish
Ask apprentices how the program is going, from their mentor and the work to pay and what comes next. Each question below comes with when to ask it and what the answer tells you.
What apprenticeship feedback survey questions tell you
Apprenticeship feedback survey questions ask apprentices how the split between work and study works, how good their mentor is, whether they learn real skills and how likely they are to finish. The answers warn employers and training providers about an apprentice at risk of leaving while there is still time to help.
Ask every three months, from the first quarter of the program to the last, and keep each round to five to seven questions. Send the link by email or show a QR code during a training day, and send the monthly log signature as its own short form.
Each written answer is read as it arrives and given a theme, a sentiment and an urgency, so an apprentice who writes about a problem at work reaches the program coordinator first. Give each response an owner in the inbox, and add every response to a Google Sheet or post it to Microsoft Teams.

- 04Matrix
Rate each part of the apprenticeship
Rates the training, the work, the mentor and pay in one grid. The heat map shows which part pulls the program down, and row averages let you compare one intake with the next.

Who is likely to finish and what makes it hard, read together so you can help an apprentice at risk.
- 06Big scale
How likely are you to finish the program?
How likely the apprentice is to finish, from 1 to 10. Results shows the average and a column per step; give each low score an owner in the inbox the same week.

Whether the apprentice expects a job offer at the end. A falling share of yes is worth raising with employers before the final months.
- 09Ruler
How many months into the program are you?
Months into the program, on a ruler. Results shows the median and the middle half, and the answer lets you read first-quarter answers apart from final-year ones.

Tips for asking about apprenticeships
Small choices that get honest, useful answers.
- 01
Ask every quarter
Run the same short survey every three months, so a falling score shows up while the apprentice is still on the program.
- 02
Act on low finishing scores
Give every low score on finishing an owner in the inbox, and arrange a conversation with the apprentice that week.
- 03
Compare employers and workplaces
Tag the link with the employer or workplace, so mentors and workplaces each get their own scores.
- 04
Keep the log separate
Send the monthly log signature as its own short form. Mixed into a feedback survey, it turns the survey into paperwork.
