Invoice feedback survey template

Send invoices that match what the client expected to pay

Four questions for a few days after the invoice: whether it matched what the client expected, how clear the line items were, how easy it was to pay and what they had to ask. Usertick reads every written answer and files it under invoices and billing or pricing.

What this invoice feedback survey tells you

An invoice feedback survey finds the billing surprises that sour a good project. This template asks four questions in under a minute: whether the invoice matched what the client expected, how clear the line items were, how easy it was to pay, and whether they had to ask you about anything.

It is written for an email a few days after the invoice goes out, once the client has read it and passed it to finance. Usertick can send it to your client contacts with the first question inside the email, or you can paste its link under the invoice in your own email. It also runs as a conversation, one question at a time.

Each written answer is read as it arrives and filed under a category such as invoices and billing, pricing and quotes or scope and changes, with a type and an urgency. Results shows the share whose invoice matched, the line item rating with the share who gave it 4 or 5, and the ease of paying with the share who found it easy, each compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Yes / No

Did the invoice match what you expected?

Why ask it
A surprise on an invoice often means a scope change that nobody priced out loud. A plain yes or no shows how often that happens across your clients.
What you learn
The share that said yes, compared with the period before. Choose No to read the clarity ratings and the questions from those clients alone.
Make it yours
Keep it first, since a yes or no question sits inside the email and one tap opens the rest of the survey.
Question 2 of 4 · Stars

How clear were the line items?

Why ask it
Vague line items such as Design services make finance teams hold an invoice. A rating from Very unclear to Very clear tells you whether yours explain themselves.
What you learn
The average on the 1 to 5 scale, a bar per rating and the share who rated 4 or higher, compared with the period before.
Make it yours
When the rating drops, compare the invoice with the statement of work and name each line the way the contract does.
Question 3 of 4 · Customer effort

How easy was it to pay?

Why ask it
Payment trouble delays your cash. A customer effort scale from Very difficult to Very easy shows whether the bank details, the portal or the card link got in the way.
What you learn
The average on the 1 to 7 scale and the share who found it easy, with a hard-to-easy bar, compared with the period before.
Make it yours
Name your payment method in the label, such as How easy was it to pay by bank transfer?, when you offer only one.
Question 4 of 4 · Text · Optional

Did you have to ask us about anything?

Why ask it
Every question a client had to ask is a line the invoice could have answered, and the open question collects them in one place.
What you learn
Usertick reads each answer as it arrives, files it under a category and a theme from your list, marks it as a question, a complaint or an idea, and flags the urgent ones.
Make it yours
Pass recurring questions to whoever writes the invoices, and answer them in a note on the next invoice.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Ask the person who pays

Send it to the contact who handles the invoice, often in finance, as well as your project contact. They see different problems.

  2. 02

Wait until it has been read

Three to five days after the invoice gives finance time to open it, check it against the purchase order and try to pay.

  3. 03

    List the scope changes

When No answers follow projects with change requests, list each change and its cost on the invoice, with the date it was agreed.

  4. 04

    Start with first invoices

The first invoice is where a client meets your billing. Send the survey after every first invoice, and after any invoice with extra work on it.
