Subscription box survey template

Hear how this month's box landed, before subscribers decide to cancel

Four questions for a few days after the box arrives: a rating for the box, whether the selection felt too predictable or too adventurous, how often subscribers want a box and what they want next. Send it by email or share the link, and Usertick reads every written answer for you.

What this subscription box survey tells you

A subscription box survey asks subscribers about the box they just opened, so the next one is better and fewer of them cancel. This template asks four questions in about a minute: a star rating for this month's box, how the selection felt from too predictable to too adventurous, how often they would like a box and an open question about the next one.

It is written for an email three days after the box arrives, once subscribers have tried what was inside. Usertick sends it to your subscriber list now or on a date up to 30 days ahead, and the star rating can be answered inside the email. Usertick has no recurring sends, so schedule each month's email by hand and turn off Skip people who already received this survey, so the same subscribers are asked again.

Every written answer is read as it arrives and filed under a category from your list, such as product quality or shipping and delivery, with a type, an urgency and a customer risk reading, so a subscriber who sounds ready to cancel stands out. Results shows the rating, the selection balance and the preferred frequency, each compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

How satisfied are you with this month's box?

Why ask it
One rating per box gives your curation team a month-by-month line, and it can be answered with one tap inside the email.
What you learn
Results shows the average rating, a bar per rating and the share who rated 4 or higher, against the period before, so a weaker month shows up beside the last one.
Make it yours
Name the month or the theme of the box in the email subject, so subscribers rate the right one.
Question 2 of 4 · Multiple choice

How did this month's selection feel?

Why ask it
Satisfaction alone doesn't say what to change. A box can disappoint because it repeats last month or because it strays too far, and the two need opposite fixes.
What you learn
A donut shows the share of Too predictable, About right and Too adventurous, and over time as stacked shares, so you can watch the balance move from one box to the next.
Make it yours
Select Too predictable and read those subscribers' open answers to find the products they are tired of.
Question 3 of 4 · Dropdown · Optional

How often would you like to get a box?

Why ask it
Some cancellations are about pace, because a box every month can pile up. Offering other frequencies tells you whether a slower plan would keep subscribers you would otherwise lose.
What you learn
A donut shows the share and count of each frequency. Select Every two months to see how those subscribers rated this month's box.
Make it yours
List only frequencies you can actually offer, and add a skip-a-month option if your plans allow it.
Question 4 of 4 · Text · Optional

What would you like to see in your next box?

Why ask it
Subscribers know what they want next: a flavor, a size, a product they saw on your site. Their ideas are the cheapest product research a box can get.
What you learn
Each answer appears with its sentiment and theme, filterable by theme, with up to 8 themes and their trends, so requests that repeat across months rise in the chart.
Make it yours
Read the answers before you finalize the next box, and mention one subscriber idea on the next box's card so people see their answers used.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Reuse one survey every month

Keep one survey so each card compares with the month before. Turn off Skip people who already received this survey before each send, and schedule the next send when the box ships.

  2. 02

Wait until the box is tried

Three days after arrival gives subscribers time to open and try the items. Schedule the send from your delivery dates, since Usertick doesn't send on a delivery event.

  3. 03

    Act on cancel signals

Each response gets a customer risk reading. On every plan, a workflow can post every response read as at risk or about to leave to Slack or Microsoft Teams, so someone reaches out before the next billing date.

  4. 04

    Share the trend with curation

Put the rating and the selection chart on a dashboard the curation team opens each month, before they pick the next box.
