Restaurant feedback survey template

Hear what diners thought, while the meal is still fresh

Four questions for a QR code on the receipt or an email after a booking: how the meal was, how the food, service, atmosphere, wait and value rated, and whether guests would recommend you. Use it as it is or change any question, and Usertick reads every written answer for you.

What this restaurant feedback survey tells you

A restaurant feedback survey asks diners about the meal while they can still name the dishes and the server. This template keeps it to four questions and just over a minute: a face for today's meal, a grid for food, service, atmosphere, waiting time and value for money, a recommendation score and one open question about what to do better.

It is written for a QR code printed on the receipt or a table card, which opens the survey on the guest's phone at the table or on the way home. The same survey goes out by email from Usertick to guests on your booking list, imported as contacts, and it can run as a conversation that asks one question at a time.

Every written answer is read as it arrives and filed under a category from your list, such as staff and service or delays and disruptions, with a type and an urgency. Results shows the share of each face, a heat map for the five parts of the visit and the NPS, each compared with the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 ยท Emoji

How was your meal today?

Why ask it
A face is the quickest answer a diner can give with a phone in one hand, so it opens the survey. It is also one of the types that sit inside the email, so guests who booked can answer with one tap.
What you learn
The faces from Very bad to Very good, each with its share, against the period before. Select Bad or Very bad to filter the page to unhappy diners and read what they wrote.
Make it yours
Keep the word today in the label, so regulars rate this one visit.
Question 2 of 4 ยท Matrix ยท Optional

How would you rate each of these?

Why ask it
A bad evening usually has one cause. The grid separates the kitchen from the floor and the room, and adds the wait and the bill, so you know which team to talk to.
What you learn
A heat map with a row for each part and its average, so slow service on a busy night or a dip in value for money stands out. Compare it with the period before after you change the menu or the staffing.
Make it yours
Add a row for what your guests mention often, such as drinks or noise, and keep the grid short, since on a phone it shows one row at a time.
Question 3 of 4 ยท NPS

How likely are you to recommend us to a friend?

Why ask it
Word of mouth fills tables, and this score tells you whether a guest is likely to send friends your way after this visit.
What you learn
The NPS, a bar of detractors, passives and promoters, the split by device and a score line over time, so you can see whether a new menu or a new chef moved it.
Make it yours
Select the detractors in Results and read their answers to the last question together, before you look at anything else.
Question 4 of 4 ยท Text ยท Optional

Is there anything we could do better?

Why ask it
The grid shows what scored low; the open question tells you why, in the guest's words, from a cold plate to a table by the kitchen door.
What you learn
Usertick reads each answer as it arrives, files it under a category and a theme, marks it as a complaint, an idea or praise, and flags urgent ones such as a wrong charge on the bill. Results lists the answers by theme, with up to 8 themes and their trends.
Make it yours
Keep it optional and last, so a guest who taps a face and leaves still counts.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Print the code where guests look

Put the QR code on the receipt, the bill folder or a table card. The code points to the live survey, so you can change a question without reprinting anything.

  2. 02

    Use one survey per location

If you run more than one restaurant, duplicate the survey for each one, so Results shows every place on its own.

  3. 03

    Answer complaints during service

On every plan, a workflow posts low faces and their comments to Slack or Microsoft Teams, so the manager on shift sees them while the evening is still running.

  4. 04

    Email guests who booked

Import each day's bookings with email addresses as contacts and send the survey the next morning. The first question sits inside the email, so a tap starts the survey.

Frequently asked questions

At the end of the meal or on the way home, when they still remember what they ate. A QR code on the receipt reaches guests at the table, and an email the next morning reaches guests who booked.

As few as you can manage, since most guests answer on a phone. This template asks four and takes just over a minute, and only the face and the recommendation score are required.

Yes. Add a question that asks which meal it was, or run one survey for each service. Selecting an answer in Results filters the whole page to the guests who gave it.

Yes. The survey asks for no name or email, and with Anonymous responses on, no user ID, traits or visitor ID are stored with the answers (the country and a salted hash of the IP address are kept). That holds for surveys opened by link or QR code; a survey sent through Usertick email stays tied to the guest's contact.

Select the low faces in Results to read those guests' comments together, and check the grid for the row that scored lowest. When a guest who booked leaves a complaint, give the response an owner in the inbox so someone replies.

Yes. Every template comes with every plan, the free plan included, and the free plan covers 150 responses a month across your surveys.

Yes. The template becomes your own survey, so reword any question, add or remove questions, and change the options and the logic. The gallery stays as it is.
