Spa feedback survey template

Learn how each treatment felt, and who will book the next one

Three questions for guests after a treatment: how satisfied they were, how the therapist, the room, booking and value rated, and whether they would book again. Add an open question for comments, and Usertick reads every written answer for you.

What this spa feedback survey tells you

A spa feedback survey asks guests about a treatment while they still feel its effect. This template takes under a minute: a satisfaction score for the treatment, a grid for the therapist, the treatment room, booking and value for money, and a yes or no on booking another treatment.

It is written for an email on the day of the treatment, sent by Usertick to the guests on your appointment list, where the first question can be answered inside the message. The same survey runs as a link in your own booking confirmation, as a QR code at reception or in the relaxation room, or as a conversation that asks one question at a time.

The template has no open question, so add one at the end when you want comments; Usertick then reads every written answer and files it under a category from your list, such as staff and service or price and fees. Results shows the treatment score with the share who rated 4 or higher, a heat map for the four parts and the share who would book again, each against the period before.

Send it from Usertick to a contact list, with the first question inside the email. A tap on an answer opens the rest of the survey with that answer already kept. Usertick sends 50 emails a month on the free plan, or connect your own sender.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Stars

How satisfied were you with your treatment?

Why ask it
The treatment is what the guest paid for, so its score comes first. Asked the same day, it records how the guest felt walking out.
What you learn
The average out of 5, a bar for each rating and the share who rated 4 or higher, against the period before. Select a rating to filter the page and see how those guests rated their therapist.
Make it yours
Keep it first, so guests can answer it inside the email with one tap.
Question 2 of 3 · Matrix · Optional

How would you rate the following?

Why ask it
A treatment can disappoint for reasons outside the therapist's hands, such as a cold room, a booking that took three calls or a price that felt high. Four rows separate the person from the place and the price.
What you learn
A heat map with a row for each part and its average, so value for money scoring below the therapist points to your prices or packages before your team.
Make it yours
Share therapist scores with each therapist one to one, and look at several weeks of answers before drawing conclusions about a person.
Question 3 of 3 · Yes / No

Would you book another treatment with us?

Why ask it
Whether a guest would book again is the closest the survey gets to your next appointment. A yes or no is easy to answer and easy to act on.
What you learn
The share who said yes, compared with the period before. Select No to filter the page to those guests and see which part of the grid they rated lowest.
Make it yours
Add skip logic so a No goes to a new open question about what would bring them back; skip logic works on every plan.

What the answers show

Results turns each question into numbers you can follow week by week.

How satisfied were you with your treatment?

The average rating and the share who gave 4 or 5 stars, against the period before.

How would you rate the following?

Each row's answers side by side, so the part that needs work stands out.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Send it the same day

Send the survey a few hours after the treatment, while the guest still feels it. Import the day's appointments as contacts and schedule the send for the late afternoon.

  2. 02

    Keep health questions out

Ask about the experience and leave out medical conditions. Usertick's terms don't allow health records or medical details in surveys.

  3. 03

    Follow up on every No

Give each response with a No an owner in the inbox, so the spa manager can call the guest and offer to put things right.

  4. 04

    Compare treatments on your menu

Add a question that names the treatment, such as a massage or a facial, then select each one in Results to compare scores across your menu.
