Spa, gym and pool survey questions

Spa and gym questions that show what makes guests book again

Ask guests about the treatment, the gym and the pool, from booking to how relaxed they felt afterward. Each question below comes with when to ask it and what the answer tells you.

What spa, gym and pool survey questions tell you

Hotel spa, gym and pool survey questions ask guests about the facilities that set a stay apart: how a treatment felt, how the therapist and the treatment room rated, how easy booking was and what the gym or the pool was missing. A spa guest who leaves less relaxed than they arrived rarely books a second treatment, and the reason tends to be a detail you can fix.

Ask about a treatment within a few hours of it, with a QR code at the spa desk or a link in an email that evening. Ask about the gym and the pool in the survey after checkout, and only to guests who used them. Pick three or four questions per visit, ending with the open question about which treatment to add.

Each written answer gets a category, a theme, a sentiment and an urgency as it arrives, so a complaint about a cold treatment room reaches the spa manager the same day and a request for a new treatment is filed as an idea.

The 13 spa, gym and pool questions, with when to ask each

Grouped by what they tell you. Use them as they are or change the wording.

The treatment itself

Ask how the treatment felt and how each part of the spa rated, then whether the guest would book it again.

  1. 01Emoji

    How did the treatment feel?

How the treatment felt, on a row of faces. Ask it first, while the guest is still relaxed; Results shows each face with its share, compared with the period before.

  2. 10Big scale

How relaxed did you feel afterward?

How relaxed the guest felt afterward, from 1 to 10. It is the outcome guests pay for, so read the average beside the therapist row of the grid.

  3. 02Matrix

    Rate the spa on each

Rates the therapist, the treatment room, booking and cleanliness on one grid. The heat map shows which part let a treatment down, so the feedback reaches the right person in the spa team.

  4. 03Yes / No

Would you book the treatment again?

A yes or no on booking the treatment again. Select No in Results to read those guests' grid ratings and see whether the therapist or the room was the problem.

Booking and timing at the spa

These show whether the spa is easy to get into, from booking a slot to waiting for it to start.

  1. 07Customer effort

How easy was it to book a treatment?

A customer effort score for booking a treatment, on a 7-point scale. Results shows the average and the share who found it easy; a low score can explain empty slots on a busy day.

  2. 12Ruler

How many minutes did you wait for a treatment?

Minutes the guest waited for the treatment to start, on a ruler from 0 to 30. Results shows the median and the middle half; a long wait can undo the calm a treatment is meant to bring.

  3. 08Dropdown

When did you use the spa?

When the guest used the spa, with an option for guests who didn't. Add logic to skip the spa questions for anyone who picks I did not, and compare times of day in Results.

  4. 11Yes / No

    Were the hours convenient?

A yes or no on opening hours. Select No in Results and read the open answers for the hours guests wanted, before you change staff schedules.

The gym and the pool

Ask guests who used the gym or the pool how they rated them and what was missing.

  1. 04Stars

How would you rate the gym?

A star rating for the gym. Results shows the average and the share who rated 4 or higher; read it with the missing equipment question to see what to buy first.

  2. 05Multiple choice

What was missing in the gym?

Lets guests pick what the gym was missing, with several picks allowed. Results shows a bar per option, which works as a shopping list; keep Nothing so content guests aren't pushed into a complaint.

  3. 06Stars

How would you rate the pool?

A star rating for the pool. A falling average may point to crowding at peak hours or the water temperature; read the open answers from the same guests for the cause.

Before the next treatment

Collect ideas for new treatments, and use the signature in a separate form before a treatment.

  1. 09Text

    Which treatment should we offer?

An open question for a treatment you don't offer yet. Each answer is filed as an idea with a theme, so repeated requests for the same treatment stand out.

  2. 13Signature

Sign to confirm your health form is accurate

A signature confirming the guest's health form before a treatment. Use it in a pre-treatment form, keep any health details on your own intake paperwork, and find the signatures in a gallery in Results. Signature is on every plan, 1 per survey on Free.

Tips for asking about spa, gym and pool

Small choices that get honest, useful answers.

  1. 01

    Ask soon after the treatment

Send the survey within a few hours, or put a QR code at the spa desk. By the next day, a massage blurs into the rest of the stay.

  2. 02

Keep the spa and the gym apart

Ask treatment questions in a spa survey and gym or pool questions after checkout, so each guest sees only what they used.

  3. 03

Follow up on a low face the same day

Give each low treatment rating an owner in the inbox, so the spa manager can call before the guest checks out.

  4. 04

Leave health details off the survey

Collect the health form on your own intake paperwork, and use the survey for the experience: the therapist, the room and how the guest felt.
