Wedding venue inquiry form template

Turn wedding inquiries into tours, with the details you need for the first call

Four questions for couples on your weddings page: the date they have in mind, about how many guests, what matters most for their day and how to reach them. Add an open question for their plans in their own words, and Usertick reads every written answer for you.

What this wedding inquiry form tells you

A wedding venue inquiry form collects what your events team needs before the first call: the date, the size and what the couple cares about most. This template asks four questions in about a minute: the date they are considering, about how many guests they expect, a ranking of the setting, food and drinks, price, capacity and rooms for guests, and how to reach them.

It is written for your weddings page, where it can sit inside the page under your packages, open from a side button, or appear as a popup once a visitor has scrolled through the page. The same form runs as a link in your replies to email inquiries and on your social profiles, or as a QR code in a printed brochure or on your stand at a wedding fair.

Each inquiry lands in the inbox, where you give it an owner, a status and notes, and the email from the Contact answer becomes the couple's email there. Results shows a calendar of the dates couples picked, the median guest count with its middle half, the average place of each priority and a table of names and emails.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Date · Optional

What date are you considering?

Why ask it
The date decides everything else: whether you are free, which season's prices apply and how soon to call. It is optional, since many couples haven't settled on one yet.
What you learn
A calendar of the dates couples picked, so you can see which months and Saturdays are in demand before you set next year's prices.
Make it yours
Set the earliest date to today and the latest a few years ahead, so no couple picks a date in the past.
Question 2 of 4 · Number · Optional

About how many guests are you expecting?

Why ask it
The guest count tells you which rooms fit and whether the inquiry suits your venue at all. The word about takes the pressure off couples who are still writing the list.
What you learn
The median guest count and the middle half of answers, so you know the size of wedding your page attracts and whether it matches your rooms.
Make it yours
Reply first to the inquiries that fit your rooms, and send the rest a short note with the capacity you can offer.
Question 3 of 4 · Ranking · Optional

Rank what matters most for your day

Why ask it
Couples weigh the setting, the food, the price, the capacity and the guest rooms differently, and the first call goes better when you know which one leads. Ranking makes them choose.
What you learn
The average place of each priority across all inquiries, so you see whether couples come to you for the setting or for the price.
Make it yours
Open each call with the couple's top priority, and use the ranking across inquiries to decide what your weddings page shows first.
Question 4 of 4 · Contact · Optional

How can we reach you?

Why ask it
Without a way to reach the couple, the inquiry goes nowhere. It comes last, so couples have shared their plans before they give their details.
What you learn
A table of names and emails, and each email becomes the couple's email in the inbox, where the response gets an owner, a status and notes.
Make it yours
Turn on the phone and message fields, and make the email field required, so every inquiry can be answered.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Place it inside the weddings page

Embed the form under your packages, where it shows as soon as the page loads, so couples who read that far can inquire without leaving the page.

  2. 02

    Reply on the same day

On every plan, a workflow posts each new inquiry to the events team's channel in Slack or Microsoft Teams, so someone replies while the couple is still comparing venues.

  3. 03

    Give every inquiry an owner

Assign each response to a coordinator in the inbox and update its status as the conversation moves on, from first call to booked tour.

  4. 04

Bring the QR code to wedding fairs

Print the code on your stand and brochure, and use a separate survey for the fair when you want to count those inquiries apart in Results.
