A System Usability Scale survey asks people to rate ten fixed statements about a product, alternating positive and negative, and turns the answers into one usability score from 0 to 100. This template keeps the standard wording on a five-point scale from strongly disagree to strongly agree and adds an open question on what would make the product easier to use. It takes about a minute and a half.
It is written for a link you send at the end of a usability test session, or an email Usertick sends once a quarter to active users on a contact list. It also runs as a popup in your web app for signed-in users, where the statements show one row at a time. Pick one moment and keep it, since a score from a test session and a score from a quarterly email describe different experiences.
Usertick reads each written answer, files it under a category from your list, such as ease of use or speed and reliability, and gives it a type and a sentiment. Results shows a heat map of the ten statements with an average for each. Results doesn't compute the SUS score itself: from the averages, take each odd statement minus 1 and 5 minus each even statement, add the ten numbers and multiply by 2.5.
