Therapy and well-being survey questions

Therapy feedback questions that show how sessions fit the people who come

Ask clients how sessions are going, from comfort with the therapist to the format and timing that suit them. Each question below comes with when to ask it and what the answer tells you.

What therapy feedback survey questions tell you

Therapy feedback survey questions ask clients about the practice around their sessions: whether a session covered what they needed, how comfortable they feel with their therapist, which format and rhythm suit them and whether they know where to turn in a crisis. A client who feels a poor fit may stop booking without saying why, and a short question after a session gives them an easier way to say it.

Ask after a session every few weeks at most, by email or by a link your practice sends, and pick three or four questions about the sessions themselves. Several questions in this topic ask about mood, sleep and energy, which are health details: Usertick doesn't sign business associate agreements, so keep those in your clinical record and keep the survey to the experience of care.

Turn on anonymous responses for website, link or QR code surveys, and Usertick stores no user ID, traits or visitor ID with the answer; surveys sent through Usertick email are not anonymous. Each written answer gets a category, a theme, a sentiment and an urgency, so a response that needs a quick reply stands out in the inbox.

The 13 therapy and well-being questions, with when to ask each

Grouped by what they tell you. Use them as they are or change the wording.

How sessions are going

Start with whether the session covered what the client needed and how comfortable they feel, the questions a therapist can act on next week.

  1. 04Yes / No

Did today's session cover what you needed?

A yes or no on whether the session covered what the client needed. Select No in Results to filter the page and see how those clients rated comfort with their therapist.

  2. 05Stars

How comfortable do you feel with your therapist?

A star rating for comfort with the therapist. Results shows the average and the share who rated 4 or higher; a low rating is a reason to offer, gently, a change of therapist.

  3. 09Emoji

How do you feel about coming to sessions?

How the client feels about coming to sessions, on a row of faces. It reflects the whole practice, from booking to the waiting room, beyond the session itself.

  4. 02Big scale

How much progress do you feel you are making?

How much progress the client feels, from 1 to 10. Results shows the average and a column per step; keep the wording on the sessions, so clients don't describe their condition to explain a score.

Format and timing

These questions show which rhythm and format suit clients and what helps between sessions, which shapes how you plan rooms and hours.

  1. 03This or that

    For sessions you prefer

Two choices side by side: weekly or every two weeks, in person or video. Results shows a split bar per pair, which helps when you plan therapist hours and rooms.

  2. 08Multiple choice

What has helped most between sessions?

The one thing that helped most between sessions. Results shows each share in a donut, and a large share of Nothing yet tells you where clients need more support between appointments.

  3. 06Text

What would you like to focus on next time?

An open question about the next session. Clients may answer with personal details, so reword it toward the session itself, such as format or pace, and add a placeholder asking them to leave out health details.

Crisis help

Ask whether clients know where to turn and end every survey with the way to reach help, whatever else they answered.

  1. 11Yes / No

Do you know how to reach help in a crisis?

A yes or no on knowing how to reach crisis help. Every No is worth a follow up with your crisis information, and the share that said yes shows whether your materials reach clients.

  2. 13Call to action

If you need to talk to someone now, help is available

A call to action with a button that opens your crisis or support page. Put your own line and hours on that page, and place this question last so every client sees it.

Mood questions for your clinical record

These questions record mood, energy and sleep, which are health details, so keep them in your clinical tools and out of a Usertick survey.

  1. 01Mood dial

    How are you feeling today?

A mood dial for how the client feels today. Mood is a health detail, and Usertick doesn't sign business associate agreements, so track it in your clinical record and leave it out of this survey.

  2. 10Slider

How much energy do you have today?

Energy on a slider from 0 to 100. Energy is a health detail like mood and sleep, so keep it in your clinical record and out of this survey.

  3. 07Ruler

How many hours did you sleep last night?

Hours of sleep, on a ruler. Sleep is a health detail, so leave this question out of a Usertick survey and record sleep in your clinical tools instead.

  4. 12Matrix

Over the last week, how often did you feel

A grid for how often the client felt calm, anxious, rested or hopeful. These are health details that belong in your clinical tools, so leave the grid out of a Usertick survey.

Tips for asking about therapy and well-being

Small choices that get honest, useful answers.

  1. 01

Keep the survey on the sessions

Usertick doesn't sign business associate agreements, so ask about comfort, format and booking, and leave mood, sleep and symptoms to your clinical record. Add a line to every open question asking clients to leave out health details.

  2. 02

    Ask every few weeks

A survey after every session wears thin. Ask after the third session and then every month or two.

  3. 03

    Always show crisis help

End every survey with the call to action that links to your crisis information, whatever the client answered before it.

  4. 04

    Offer anonymous answers

Turn on anonymous responses for link or QR code surveys, so clients can speak freely about their therapist. Surveys sent through Usertick email are not anonymous.

Add these questions to a survey

From the library to the first answers.

  1. 1

Paste questions reads up to 10 questions from any text on Free, 100 on Starter and 200 on Pro and above, keeps your wording and picks an answer type for each.

  3. 3

    Choose where it runs

Show the survey on your website, send it by email, share its link and QR code, or switch it to a conversation.

  4. 4

    Read the answers

Results counts every score and choice, and Usertick reads each written answer and files it by topic, on every plan.
