Counseling feedback survey template

Check in after every session, in the client's own words

Four questions for after a counseling or coaching session: how the session felt, how heard the client felt, whether the pace was right and what they want to bring up next time. Use it as it is or change any question, and Usertick reads every written answer for you.

What this counseling feedback survey tells you

A counseling feedback survey asks clients about the session itself: how it felt, whether they felt heard, whether the pace suited them and what they want to raise next time. This template takes under a minute and asks about the working relationship, never about the client's history or what was discussed.

It is written for the hours after each session, sent by Usertick email to the clients seen that day or as a link in the message your booking system sends. Clients answer in their own time, away from the room, which can make it easier to be honest about a session that felt off.

Results shows the faces from difficult to very helpful with each share, the average on the feeling-heard scale with a column per step, and the split of too fast, about right and too slow. The written answer is read as it arrives and filed under a category such as staff and communication, with a type and an urgency.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Mood dial

How did today's session feel?

Why ask it
The dial's faces run from difficult through heavy and okay to very helpful, because a useful session can still feel heavy. The client turns to the face that matches the session.
What you learn
Faces from unhappy to happy, each with its share, against the period before. Read Difficult and Heavy beside the feeling-heard scale, since a heavy session where the client felt heard can be a good one.
Make it yours
In the email, a dial shows as a button that opens the survey, and the rest of the survey follows on the hosted page.
Question 2 of 4 · Big scale

How much did you feel heard?

Why ask it
Feeling heard sits at the center of the working relationship between a counselor and a client, and it is something a counselor can change from one session to the next.
What you learn
The average and a column per step from 1 to 10, compared with the period before. Select the low columns to read what those clients want to bring up next time.
Make it yours
Keep the ends at Not at all and Completely, so a 10 always means the client felt fully heard.
Question 3 of 4 · Multiple choice

The pace of the session was

Why ask it
A session that moves too fast or too slowly can leave a client feeling rushed or stuck, even when the content was right. Three options make the answer one tap.
What you learn
A donut with the share and count of too fast, about right and too slow, and the stacked shares over time.
Make it yours
With several counselors, add a Dropdown with each counselor's name, since Results doesn't split answers by the tag on a link.
Question 4 of 4 · Text · Optional

Is there anything you'd like to bring up next time?

Why ask it
The last question invites clients to set part of the next session's agenda, which tells the counselor what to come back to.
What you learn
Each answer with its sentiment and theme, filterable by theme, with up to 8 themes and their trends. Usertick also marks each one as a question, an idea, a complaint or praise and flags the urgent ones.
Make it yours
Use the placeholder to ask for a topic in a few words, so clients can raise a subject without writing out the details.

What the answers show

Results turns each question into numbers you can follow week by week.

The pace of the session was

The share of each answer with its count. Select an answer to narrow the whole page to those people and read what else they said.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Point clients to urgent help

Add a message screen at the start or the end that says where to get help right away, since your team may not read an answer the moment it arrives.

  2. 02

    Ask after every session

Turn off Skip people who already received this survey, which is on by default, so each client can answer after every session.

  3. 03

    Offer a route without names

A survey sent through Usertick email is tied to the client's contact. For feedback on the service as a whole, share a second copy by link or QR code with anonymous responses turned on.

  4. 04

    Keep session content out

Ask how the session felt, never what was discussed. Usertick's terms don't allow health records or medical details in surveys, and Usertick doesn't sign business associate agreements.
