Campus dining survey template

Hear what students think of the food, right at the table

Three questions to share with a QR code on the tables: how satisfied students are with the food, a rating for quality, price, waiting time and dietary options, and the dish they would add. Use it as it is or change any question, and Usertick reads every written answer for you.

What this campus dining survey tells you

A cafeteria survey asks students about campus food while they are eating it. This template takes about a minute: a satisfaction rating, a rating for quality, price, waiting time and dietary options, and one open question on the dish they would like added to the menu.

It is written for a QR code on cafeteria tables, tray returns and the line, where students scan it with their phone during a meal. The same link can go in the dining newsletter or on the menu page of your website, and the survey can open as a conversation that asks one question at a time.

Every written answer is read as it arrives, given a theme and a sentiment and filed under a category from your education list, with a type and an urgency. Results shows the satisfaction rating and the share who rated it 4 or higher, and a heat map of the four ratings with their averages, each against the period before, so a new menu shows in the numbers.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Stars

How satisfied are you with the food on campus?

Why ask it
One rating gives the dining hall a number to follow week by week, so a new menu or a new supplier shows in the score.
What you learn
The average out of 5, a bar per rating and the share who rated the food 4 or higher, against the period before.
Make it yours
Keep it first and required, so a student who only scans and taps still leaves a score.
Question 2 of 3 · Matrix · Optional

How would you rate the following?

Why ask it
Food can taste good and still cost too much or take too long. Four rows separate quality, price, waiting time and dietary options, and a different team can fix each one.
What you learn
A heat map with one row per item and its average, so a slow lunch line or thin vegetarian choices stand out against the food itself.
Make it yours
Add a row for what you are changing, such as a new salad bar, and remove it once its rating settles.
Question 3 of 3 · Text · Optional

What dish would you like us to add?

Why ask it
Asking for a dish gets concrete ideas the kitchen can try, and the answers can point to dietary needs the menu misses.
What you learn
Usertick gives each answer a sentiment and a theme, and groups the answers into up to 8 themes with their trends, so requests such as more vegetarian dinners or gluten-free bread show as one theme.
Make it yours
Put the most requested dish on the menu for a week, and say on the table card that it came from student answers.

What the answers show

How would you rate the following?

Each row's answers side by side, so the part that needs work stands out.

Every written answer, read for you

Each answer arrives with its category, theme, type and urgency, so the urgent ones reach your team first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Put codes where people wait

Place the QR code on tables and in the line. The code points to the live survey, so changing a question never needs a new print.

  2. 02

Use one survey per dining hall

Results doesn't split answers by a link tag, so use a copy for each dining hall, or add a question that names the hall, when you run more than one.

  3. 03

Add your logo to the code

Turn on Logo in the code before you download the PNG, so students recognize the code as the dining team's.

  4. 04

    Read it after menu changes

Compare the weeks after a new menu with the weeks before. Every card in Results shows the change from the period before.
