Camp feedback survey template

Hear how camp went for every family, and who will sign up again

Three quick questions for parents and guardians the weekend after camp: how much their child enjoyed it, a rating for safety, activities, communication and food, and how likely they are to sign up next year. Use it as it is or add an open question, and Usertick reads every written answer for you.

What this camp feedback survey tells you

A camp feedback survey for parents asks families how camp went while the stories are still coming home. This template takes under a minute and goes to parents and guardians: how much their child enjoyed camp, a rating for safety, activities, communication with parents and food, and how likely they are to sign up again next year.

It is written for an email to the parents and guardians on your camp list the weekend after camp ends, scheduled ahead so it arrives on Saturday morning. The same template runs as a link in your own end-of-camp email or on the camp page of your website, or as a conversation that asks one question at a time.

Results shows the share of each face for enjoyment, a heat map of the four ratings with their averages, and the share of families likely to sign up again, each against the period before. Selecting Unlikely filters the whole page to those families. Add an open question, and Usertick reads every written answer and files it under a category with a type and an urgency.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 ยท Emoji

How much did your child enjoy camp?

Why ask it
Enjoyment is the first thing parents hear about at home, and a face is a quick way to pass it on.
What you learn
Results shows each face, from unhappy to happy, with its share, against the period before.
Make it yours
The faces are labeled Very bad to Very good while the scale ends read Not at all and Loved it, so relabel the faces to match the question before you send it.
Question 2 of 3 ยท Matrix ยท Optional

How would you rate the following?

Why ask it
Four rows cover what parents weigh when they choose a camp: safety, activities, how well you kept them informed and the food. A low row shows what to fix before next year.
What you learn
A heat map with one row per item and its average, so a low safety or communication rating stands out against the others.
Make it yours
Add rows for what your camp offers, such as transportation or swimming lessons, and keep the same five columns.
Question 3 of 3 ยท Multiple choice

How likely are you to sign up again next year?

Why ask it
Whether families plan to come back is the clearest sign of how camp went, and it gives you an early count for next year.
What you learn
A donut with the share and count for very likely, likely, not sure and unlikely. Select Unlikely to see how those families rated safety and activities.
Make it yours
Send it by email, so each answer is tied to the parent's contact and you can send early sign-up details to families who chose Very likely.

What the answers show

Results turns each question into numbers you can follow week by week.

How likely are you to sign up again next year?

The share of each answer with its count. Select an answer to narrow the whole page to those people and read what else they said.

How would you rate the following?

Each row's answers side by side, so the part that needs work stands out.
