Event accessibility survey template

Find the barriers attendees met, and remove them before the next event

Two questions for after your event: a rating for getting around the venue, seating, captions and audio, quiet spaces and signs, and what would have made the event easier to attend. Use it as it is or change any question, and Usertick reads every written answer for you.

What this event accessibility survey tells you

An event accessibility survey asks every attendee how easy it was to take part, from moving between rooms to following the talks. This template takes about a minute: one rating grid for five parts of access and one open question about what would have helped. It covers the barriers that are easy to miss from the organizer's side, such as a ramp behind a locked door or captions that stopped after lunch.

It is written for an email to all attendees after the event, sent by Usertick to your attendee list, so people who never asked for support can still tell you what got in their way. The same template also runs as a link in your own wrap-up email, or as a QR code on signs at the help desk and in the quiet room. Answers sent through Usertick email are tied to each attendee's contact, so your access lead can reply to a specific problem.

Every written answer is read as it arrives and filed under a category such as venue and rooms, with a type, a sentiment and an urgency. Results draws a heat map of the five ratings with their averages, and selecting a low rating filters the page to those attendees and what they wrote.

The 2 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 2 · Matrix · Optional

How would you rate the following?

Why ask it
Access has several parts, and an event can get the ramps right and the captions wrong. A grid of five ratings shows which part to fix first. Every row uses the same scale from Poor to Excellent, so the five parts compare directly.
What you learn
A heat map with a row for each part and its average, compared with the period before. A Not applicable column, if you add one, keeps attendees who didn't use captions or the quiet room out of those averages.
Make it yours
Add rows for what you offered, such as sign language interpretation or a ramp to the stage, and add a Not applicable column.
Question 2 of 2 · Text · Optional

What would have made the event easier for you to attend?

Why ask it
The ratings show where access fell short. The open question tells you what to change, in the words of the attendees who met the barrier. Many answers name one fix, such as a sign, a seat or a microphone, that costs little next time.
What you learn
Each answer with its sentiment and theme, filterable by theme, with up to 8 themes and their trends. Usertick marks each as a complaint, an idea or a question and flags the urgent ones.
Make it yours
Ask about what to arrange, such as step-free routes or reserved seats, and leave out questions about medical conditions, which Usertick's terms don't allow. Read the answers with the row ratings beside them to see which barrier each one describes.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Send it to every attendee

Attendees who never asked for support may still have met a barrier, such as a stage they couldn't hear or a sign they couldn't read. Send it to the whole list.

  2. 02

    Keep the wording plain

Use short row names and a clear, high-contrast theme. On phones and in popups the grid shows one row at a time, which keeps each rating easy to read.

  3. 03

    Act on the urgent answers

On every plan, a workflow posts urgent answers to Slack or Microsoft Teams, so your access lead can reply while the event is still fresh.

  4. 04

    Tell attendees what changed

Say in the next invitation what you changed because of these answers, such as captions on every stage or a second quiet room.
