Session feedback survey template

Rate every talk, before attendees leave the room

Five quick questions for the end of a conference session: a rating for the session and the speaker, the level of the content, how people attended and the one thing they will take away. Use it as it is or change any question, and Usertick reads every written answer for you.

What this session feedback survey tells you

A session feedback survey asks attendees about one talk or workshop while they are still in their seats. This template takes about a minute: a rating for the session, a rating for the speaker, whether the level was right, how they attended and one takeaway in their own words.

It is written for a QR code on the last slide, so attendees scan it as the speaker wraps up and answer on their phones. The same link also works on the session page of your event app, in the chat for online viewers or under the recording, and the template can run as a popup on the session page of your event website.

Every takeaway is read as it arrives and given a theme and a sentiment, so you can see what landed without reading every line. Results shows the session and speaker averages with the share who rated 4 or higher, and selecting an answer filters the whole page to those attendees.

The 5 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 5 · Stars

How would you rate this session?

Why ask it
The session score rates the content and the format together, the part your program team controls. Asked first, while the slides are still up, it gets the most answers.
What you learn
The average out of 5, a bar for each rating and the share who rated 4 or higher, compared with the period before.
Make it yours
Ask each speaker to leave the QR code slide up for the last minute of the Q&A, so attendees can scan it before they stand.
Question 2 of 5 · Stars

How would you rate the speaker?

Why ask it
A strong speaker can carry a weak topic, and a flat delivery can sink a strong one. A separate speaker score shows which of the two happened.
What you learn
The speaker's average and the share of 4 and 5 ratings beside the session score, so a gap between the two stands out.
Make it yours
Send each speaker their own scores and takeaways after the event, so they can sharpen the talk before they give it again.
Question 3 of 5 · Multiple choice

The level of the content was

Why ask it
The level of the content explains many middling scores. Attendees who found it too basic or too advanced rate the session lower even when the speaker did well.
What you learn
The share for each level, compared with the period before. Select Too basic or Too advanced to see how those attendees rated the session and the speaker.
Make it yours
Use the answers to print a level beside each session on the next agenda, so attendees pick the right room.
Question 4 of 5 · Dropdown · Optional

How did you attend?

Why ask it
People in the room, people watching live and people catching the recording have different experiences of the same talk. This question lets you compare them in one survey.
What you learn
A donut with the share and count for in person, online live and the recording. Select one group to filter the page and compare its ratings with the others.
Make it yours
Delete the options you don't offer; for a session with no stream and no recording, remove the question.
Question 5 of 5 · Text · Optional

What's one thing you'll take away from this session?

Why ask it
Asking for one takeaway checks whether the main point of the talk landed. The answers also make honest quotes for the speaker and the next event page.
What you learn
Each answer with its sentiment and theme, filterable by theme, with up to 8 themes and their trends. Usertick also marks each answer as praise, an idea, a question or a complaint.
Make it yours
Ask the speaker for the one point they meant to make, then compare it with the themes in the answers.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Use one survey per session

Results and the inbox don't split answers by link tag, so duplicate the survey for each session and give each copy its own QR code. For a program of many short talks, add a first question that names the session and filter Results by it.

  2. 02

Show the code before the Q&A

Put the QR code on the last slide and keep it up through the questions. Attendees scan it while they wait, before they leave the room.

  3. 03

Tag the link for each place

Add ?utm_source=slide or ?utm_source=recording to the links you share. Each answer keeps its tag for workflows, Google Sheets, webhooks and the API.

  4. 04

    Keep it under a minute

Five questions fit into the gap between two sessions. Save the broader questions about the venue and the schedule for the post-event survey the next day.

Frequently asked questions

At the end of the session, before they leave the room. Put the QR code on the closing slide and give attendees a minute during the Q&A.

You can, by adding a first question that names the session, such as a dropdown of talks, and filtering Results by it. For a smaller program, duplicate the survey for each session so every talk has its own results.

Post the same link in the stream chat or under the recording. The question about how they attended keeps their answers apart from the room's when you filter.

No. Download the QR code as a PNG or an SVG and place it on the slide. The code points to the live survey, so later edits never require a new print.

Select the low scores in Results and read those attendees' takeaways and level answers. A low score with Too advanced points to the content level, which the speaker can adjust for the next audience.
