Hands-on workshop survey template

Learn what stopped people finishing, before you run the workshop again

Four questions for the last minute of a hands-on workshop: whether people finished the exercise, how the pace felt, a rating for the facilitator and what slowed them down. Use it as it is or add an open question, and Usertick reads every written answer for you.

What this workshop feedback survey tells you

A workshop feedback survey asks participants how the hands-on part went while their laptops are still open. This template takes under a minute: whether they finished the exercise, the pace, a rating for the facilitator and the things that slowed them down, from setup to Wi-Fi. It turns a vague sense that the room struggled into the share who finished and the reasons the others didn't.

It is written for a QR code or a short link on the last slide, answered before people pack up. The same template also runs as a link in your follow-up email or the event app, or as a popup on the workshop page of your course or event website.

Results shows the share who finished, the split between too fast, about right and too slow, the facilitator's average and a bar for each blocker with the typed Other answers listed. Add an open question and Usertick reads each written answer, filing it under a category with a type and an urgency.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Yes / No

Did you finish the exercise?

Why ask it
Finishing the exercise is the outcome a hands-on workshop is built around. A yes or no gives you a completion share to improve from one run to the next.
What you learn
The share that said yes, compared with the period before. Select No to filter the page to those participants and see their pace and blockers.
Make it yours
When the exercise has stages, ask which stage they reached with a multiple choice question in place of the yes or no.
Question 2 of 4 · Multiple choice

The pace of the workshop was

Why ask it
Pace is the main lever a facilitator controls during the session. The answer tells you whether to cut steps, add time or add a stretch goal for fast finishers.
What you learn
The share for too fast, about right and too slow, compared with the period before. Select Too fast to see how many of those participants finished.
Make it yours
Read it with the first question: too fast with many unfinished means cut the exercise, and too slow with everyone finished means add a harder step.
Question 3 of 4 · Stars

How would you rate the facilitator?

Why ask it
A facilitator who walks the room and unblocks people changes the outcome as much as the material does. A separate score shows which of the two to work on.
What you learn
The average out of 5, a bar for each rating and the share who rated 4 or higher, compared with the period before.
Make it yours
Share the score with the facilitator after each run, together with the blockers participants chose.
Question 4 of 4 · Multiple choice · Optional

What slowed you down?

Why ask it
Time in a hands-on workshop goes to installing tools, a weak network or a step that wasn't clear. Naming the cause turns a slow session into a list you can fix.
What you learn
Bars with the share and count for each blocker, since participants can pick several, with every typed Other answer listed beside the chart.
Make it yours
Add the blockers specific to your workshop, such as an expired license key or a missing data file, and keep Nothing as an option.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

    Send setup steps beforehand

When Setup and installation leads the blockers, send the setup steps a few days before the next run and open the workshop with a quick check.

  2. 02

    Ask before laptops close

Show the link in the last two minutes. A short link or a QR code on the final slide opens on the laptop or phone participants already have in hand.

  3. 03

    Duplicate it for each workshop

Results and the inbox don't split answers by link tag, so give each workshop or each facilitator its own copy of the survey and its own link.

  4. 04

    Add one open question

The template has no text question. Add What would you change about the exercise? at the end, and Usertick reads each answer and files it by category and theme.
