Booth visitor survey template

Turn booth visits into follow-ups, with the reason each visitor stopped

Four questions for a tablet at your booth or a QR code on the stand: what brought each visitor over, how useful the conversation was, whether they want a follow-up and where to reach them. Use it as it is or add an open question, and Usertick reads every written answer for you.

What this trade show booth survey tells you

A trade show booth survey captures who stopped at your stand, why, and whether they want to hear from you after the show. This template takes under a minute: what brought them over, a score for the conversation with your team, a yes or no on a follow-up and a Contact question for their details.

It is written for the show floor: open the survey link on a tablet at the booth, or print its QR code on the stand and on giveaway cards so visitors answer on their own phones. The same template also runs as a link in your post-show email or as a popup on the landing page you send booth visitors to.

Results shows the share of each reason with the typed Other answers, the conversation score with the share who rated 4 or higher, the share who want a follow-up and a table of names and emails. The email from a Contact answer can become a contact in Usertick, and on every plan the HubSpot connector syncs it to your CRM.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Multiple choice

What brought you to our booth?

Why ask it
Knowing what drew each visitor tells you what to repeat at the next show: the demo, the giveaway, a referral or the product itself. Passing traffic has its own option, so it doesn't inflate the others.
What you learn
A donut with the share and count for each reason, with every typed Other answer listed. Select The demo to see how those visitors rated the conversation and how many want a follow-up.
Make it yours
Add the hooks you actually ran, such as a talk in the expo theater or a coffee bar, and keep I was just passing by.
Question 2 of 4 · Stars

How useful was your conversation with our team?

Why ask it
The conversation is the reason you paid for the booth. A usefulness score, asked straight after it, shows whether your team asked the right questions and showed the right things.
What you learn
The average out of 5, a bar for each rating and the share who rated 4 or higher, compared with the period before.
Make it yours
Select the low scores and check what brought those visitors: low scores from demo visitors mean the demo needs work.
Question 3 of 4 · Yes / No

Would you like us to follow up after the event?

Why ask it
A follow-up request is the line between a visit and a lead. Asking it outright saves your sales team from calling people who only wanted the giveaway.
What you learn
The share that said yes. Select Yes to filter the page to the visitors who want a follow-up and see why they came over.
Make it yours
Add skip logic so No ends the survey: set it on this question in the editor, and only interested visitors see the Contact question.
Question 4 of 4 · Contact · Optional

Where can we reach you?

Why ask it
The visitor types the details your team needs for the follow-up, such as name, email and company, without waiting for a badge scanner.
What you learn
A table of names and emails with the date of each answer. The email from a Contact answer becomes the visitor's email in the inbox and can create a contact.
Make it yours
Turn on the Company field and leave Phone off unless your team calls leads.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Keep a tablet at the booth

Open the survey link on a tablet with the screen lock off and hand it to each visitor after the conversation. Reload the page for the next visitor.

  2. 02

Put the code where people wait

Print the QR code on the stand, beside the demo screen and on the giveaway cards, so visitors in line can answer on their own phones.

  3. 03

Send leads on the same day

On every plan, a workflow posts each Yes to your sales channel in Slack, and from Starter a Zapier step in the same workflow sends it to Salesforce. The HubSpot connector creates the contact with the answers.

  4. 04

    Use one survey per show

Results doesn't split answers by link tag, so duplicate the survey for each show and compare the reasons and scores side by side.

Frequently asked questions

Open the survey link on a tablet at the booth, or print its QR code on the stand. The Contact question collects name, email and company, and each email can become a contact in Usertick.

Yes. On every plan, the HubSpot connector finds or creates the contact and writes the answers to it. A Zapier step sends leads to Salesforce and other CRMs, or a workflow adds each one to a Google Sheet.

The survey page needs a connection to send each answer. When it drops, a failed send retries twice and the form stays filled for another try, so keep the tablet on a phone hotspot as a backup.

As the template ships, they do, and the question is optional. Add a skip on the follow-up question so No ends the survey, and only visitors who want a follow-up see it.

Add a Consent question with a link to your terms, so each visitor ticks the box before you add them to a mailing list.

Yes. Every template comes with every plan, the free plan included, and the free plan covers 150 responses a month across your surveys.

Yes. The template becomes your own survey, so reword any question, add or remove questions, and change the options and the logic. The gallery stays as it is.

Yes, from Starter. The template is written in US English. Add languages by hand or with AI translation, and each person sees the survey in their browser's language.
