Bar feedback survey template

Hear how the night at the bar went, from a QR code on the bill

Three questions for guests at your bar or lounge: how satisfied they were, how the drinks, speed of service, atmosphere and prices rated, and what they would add to the drinks menu. Use it as it is or change any question, and Usertick reads every written answer for you.

What this bar feedback survey tells you

A bar customer feedback survey asks guests about the visit while they are still at the table or on the way out. This template takes about a minute: a satisfaction score for the visit, a grid for the drinks, speed of service, atmosphere and prices, and one open question about the drinks menu.

It is written for a QR code printed on the bill or the check presenter, which opens the survey on the guest's phone. The same link can sit on a table tent, a coaster or the drinks menu, and for a hotel lounge it can go on the in-room screen or in the guest messages your own system sends.

Every written answer is read as it arrives and filed under a category from your list, such as staff and service or price and fees, with a type and an urgency. Results shows the visit score with the share who rated 4 or higher and a heat map for drinks, service, atmosphere and prices, each against the period before.

The 3 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 3 · Stars

How satisfied were you with your visit to the bar?

Why ask it
One score for the visit lets you compare nights, events and menus over time. Asked first, it gives you the number even when a guest stops after one tap.
What you learn
The average out of 5, a bar for each rating and the share who rated 4 or higher, against the period before. Select the low ratings to filter the page and read what those guests wrote.
Make it yours
If you run themed nights or live music, add a question that names the night, so you can compare them in Results.
Question 2 of 3 · Matrix · Optional

How would you rate the following?

Why ask it
A bar can get the drinks right and still lose guests to a long wait at the counter or prices that feel steep. Four rows separate what is poured from how fast it arrives, the room and the bill.
What you learn
A heat map with a row for each part and its average, so speed of service dropping on busy nights shows up beside drinks that still score well.
Make it yours
Add a row for bar snacks if you serve them, and keep the grid short, since on a phone it shows one row at a time.
Question 3 of 3 · Text · Optional

What would you add to the drinks menu?

Why ask it
Guests know what they order elsewhere. Asking what they would add gives your bar team ideas for the next menu, from a local beer to an alcohol-free cocktail.
What you learn
Usertick reads each answer, files it under a theme from your list and marks it as an idea, a complaint or praise. Results lists the answers by theme, with up to 8 themes and their trends, so a drink asked for again and again rises to the top.
Make it yours
Read these answers before each menu change, and try the most requested drink as a special first.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Print the code on every bill

Put the QR code on the bill, the check presenter or a coaster, where guests look at the end of the visit. The code points to the live survey, so editing a question needs no reprint.

  2. 02

    Use one survey per bar

If you run more than one bar or lounge, duplicate the survey for each, so Results shows every place on its own.

  3. 03

    Watch the busy nights

Read the speed of service row after your busiest nights. When it drops while the drinks stay high, look at staffing before the menu.

  4. 04

Pass complaints to the manager on duty

On every plan, a workflow posts low scores and their comments to Slack or Microsoft Teams, so the manager can speak to the guest while they are still there.

Frequently asked questions

When the bill arrives or just after, while the visit is fresh. A QR code on the bill reaches every guest who pays.

Use one survey per bar, or add a question that names the event. A tag on the link stays with each answer for workflows, Google Sheets and webhooks, but Results doesn't split answers by tag, and the QR code in the Share step opens the plain link.

Three, in about a minute. Only the satisfaction score is required, so a guest can answer with one tap and go.

Read the written answers before you cut prices. When the drinks score high and prices low, guests may be asking for a cheaper option or an early evening offer more than for lower prices across the menu.
