Usertick
Takeout and delivery survey template

Find out how each order arrived, from the food to the bag it came in

Four questions for customers who ordered takeout or delivery: how satisfied they were, whether the order was complete and correct, how temperature, packaging and delivery time rated, and how easy ordering was. Add an open question for comments, and Usertick reads every written answer for you.

What this takeout and delivery survey tells you

A takeout and delivery survey asks what happened after the order left your kitchen. This template takes under a minute: a satisfaction score for the order, a yes or no on whether it was complete and correct, a grid for food temperature on arrival, packaging and delivery time, and a customer effort score for ordering.

It is written for the hour after the meal arrives: a QR code on a sticker on the bag or a card in the box, or an email from Usertick to the customers you import as contacts, where the first question can be answered inside the message. When your ordering website runs on your own domain, it can open there as a popup when the customer comes back. Usertick has no trigger from your delivery system, so it can't send the survey a set time after each delivery.

The template has no open question, so add one when you want comments; Usertick then reads each written answer and files it under a category from your list, such as delays and disruptions or staff and service. Results shows the satisfaction score, the share of complete and correct orders, a heat map for temperature, packaging and delivery time, and the share who found ordering easy, each against the period before.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 · Stars

How satisfied were you with your order?

Why ask it
One score for the order covers everything the customer saw, from placing the order to the last bite. It comes first, so a one-tap answer still counts.
What you learn
The average out of 5, a bar for each rating and the share who rated 4 or higher, against the period before. Select the low ratings to see which part of the grid those customers rated lowest.
Make it yours
Keep it first, so customers can answer it inside the email with one tap.
Question 2 of 4 · Yes / No

Was your order complete and correct?

Why ask it
A missing side or a wrong drink spoils a delivery, and the customer can't walk back to the counter to fix it. A yes or no tells you how often it happens.
What you learn
The share who said yes, against the period before. Select No to filter the page to those orders and see how those customers rated the rest.
Make it yours
Add skip logic so a No goes to a short open question about what was missing, and give those responses an owner in the inbox so someone offers a refund or a credit.
Question 3 of 4 · Matrix · Optional

How would you rate the following?

Why ask it
Food that left the kitchen hot can arrive cold, soggy or late. Three rows separate the food on arrival, the packaging and the delivery time, which have different fixes.
What you learn
A heat map with a row for each part and its average, so packaging scoring below delivery time points to containers before drivers.
Make it yours
If you use a delivery partner and your own drivers, add a question for who delivered, so you can compare them.
Question 4 of 4 · Customer effort

How easy was it to order?

Why ask it
Ordering is the step customers repeat next time. A scale from very difficult to very easy shows whether your menu, website or phone line makes it harder than it should be.
What you learn
The average on the 1 to 7 scale, the share who found ordering easy and a hard-to-easy bar, compared with the period before.
Make it yours
Add a question for how the customer ordered, such as your website, by phone or a delivery platform, and compare the effort scores for each.

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Put the code on the bag

Print the QR code on a sticker for the bag or a card in the box, so every customer finds it when they unpack. The code points to the live survey, so editing a question needs no reprint.

  2. 02

    Email the evening's orders

Import each evening's online orders with email addresses as contacts and schedule the send for soon after the last delivery.

  3. 03

    Fix incomplete orders first

When the share of complete and correct orders drops, check the packing step before anything else, since every missing item is part of a meal the customer paid for.

  4. 04

    Ask regulars again later

When your ordering website runs on your own domain, set the popup to show again after a number of days, so regular customers rate a recent order too.
