Volunteer feedback survey template

Hear how the shift went, the day after it ends

Four questions for volunteers after a shift: how it felt, how the briefing, supplies, coordination and appreciation held up, whether they plan to come back and what would make the next shift better. Use it as it is or change any question, and Usertick reads every written answer for you.

What this volunteer feedback survey tells you

A volunteer feedback survey asks people who gave their time how the shift actually went. This template takes about 75 seconds: a face for the shift as a whole, a grid for the briefing, the supplies, the coordination and feeling appreciated, a question on coming back and one open question on the next shift.

It is written for the day after a shift, sent by Usertick to the list of volunteers who worked it, with the first question inside the email. The same survey runs as a link you post in your volunteer group or as a QR code on the sign-out sheet, and on your website it can open on the shift sign-up page.

Every written answer is read as it arrives and filed under a category from your list, such as volunteering or communication, with a type, a sentiment and an urgency. Results shows the share of each face, a heat map for the four parts of the shift and the share who plan to return, each compared with the period before.

In an email

Send it from Usertick to a contact list, with the first question inside the email. A tap on an answer opens the rest of the survey with that answer already kept. Usertick sends 50 emails a month on the free plan, or connect your own sender.

The 4 questions, one by one

What each question asks, why it is there, what it shows you and what to change.

Question 1 of 4 ยท Emoji

How was your shift?

Why ask it
A face is the quickest honest answer after a long day on your feet, and it opens the survey with one tap.
What you learn
Results shows each face from Very bad to Very good with its share, against the period before. Select Bad or Very bad to read only those volunteers' other answers.
Make it yours
Keep it first, so in a Usertick email the volunteer taps a face inside the message and the rest of the survey opens with it kept.
Question 2 of 4 ยท Matrix ยท Optional

How would you rate the following?

Why ask it
The grid splits the shift into the parts a coordinator controls, so a low face points to a cause: a thin briefing, missing gloves or nobody in charge at the door.
What you learn
A heat map shows each row from Poor to Excellent with its average, so Feeling appreciated and Tools and supplies can be compared at a glance and against the last period.
Make it yours
Rename the rows to match your work, such as Safety briefing or Parking and check-in, and keep the grid to four or five rows.
Question 3 of 4 ยท Multiple choice

How likely are you to volunteer with us again?

Why ask it
Whether volunteers come back decides how many shifts you can fill next month. Asking plainly turns a feeling into a number you can plan with.
What you learn
A donut shows the share and count of each answer. Select Not sure or Unlikely to see how those volunteers rated the briefing, the supplies and the coordination.
Make it yours
Treat the Not sure group as the volunteers to win back, and read their answers to the last question first.
Question 4 of 4 ยท Text ยท Optional

What would make the next shift better?

Why ask it
One open question at the end catches what the grid missed, such as a shift that ran an hour late or a task nobody explained.
What you learn
Usertick reads each answer as it arrives, files it under a category and a theme from your list, marks it as a complaint, an idea or praise, and flags the urgent ones, such as a safety problem, in any language.
Make it yours
Keep it optional, so a tired volunteer can still send the first three answers.

What the answers show

Tips for this survey

Small choices that keep people answering and the answers useful.

  1. 01

Keep a list for each shift

Add the volunteers on each shift to a list in Contacts and send the survey to that list. Turn off Skip people who already received this survey for regulars you want to ask after every shift.

  2. 02

Schedule it while you close up

Set the send for the next morning before you leave the site, so the email arrives while the shift is still fresh.

  3. 03

Bring low faces to the coordinator

On every plan, a workflow posts Bad and Very bad answers to the coordinators' Slack or Microsoft Teams channel, so someone can call the volunteer back the same day.

  4. 04

Compare sites with one survey each

Results doesn't split answers by link tag, so duplicate the survey for each site or add a question that names the site, then filter Results by it.
